Job Description

Site: West Park Healthcare Centre

Department: Seniors Mental Health 

Reports to: Manager, Program Services   

Work Model: On-Site

Hours: 37.5 per week

Shifts: 8 hours

Status: Permanent Full-Time

Closing Date: Until Filled



Position Summary



The Seniors Mental Health Consultant provides psychogeriatric services and education on mental health issues to clients and families related to older adults.



Duties

  • Provide comprehensive assessment, consultation and education to geriatric/psychogeriatric clients and their caregivers. Assessments are completed in-home, community and/or a clinical setting.  
  • Participate in case conferences with Clinical Consultant (Psychiatrist)
  • Prepare written reports outlining intervention strategies
  • Establish liaisons with appropriate community agencies, organizations and committees whose objectives correspond to those of the service.
  • Ensure timely and accurate clinical documentation and completion of workload
  • Make appropriate referrals and provide guidance to clients and families as required
  • Participate in program planning and evaluation
  • Participate in the development and provision of educational materials and programs for clients, families and community service providers

Qualifications

  • University degree in Psychiatric Nursing, Social Work or Occupational Therapy
  • Masters’ preferred
  • Registration with the appropriate regulatory college
  • Minimum of 3 years clinical experience with geriatric clients and their families in a clinical community setting
  • Excellent interpersonal, communication skills (written and verbal)
  • Excellent interviewing, reporting and assessment skills
  • Strong knowledge of community resources and demonstrated ability to establish linkages with community service providers
  • Demonstrated teaching skills with families and professional service providers
  • Excellent organizational and problem solving skills
  • Valid drivers’ license and access to a reliable vehicle
  • Experience in adult education (organizing/presenting)
  • Computer skills
