Job Description - Settlement Health Navigator (Social Worker)

Association for New Canadians is a non-profit, community-based organization dedicated to


the provision of settlement and integration services for immigrants and refugees.



Job Title: Settlement Health Navigator


Location: St. John’s, NL


Position Type: 1 full-time position 


Closing date: April 09, 2025




Reporting to the Mental Health and Health Services Manager, the Settlement Health Navigator will be responsible for providing newcomers and their families with immediate essential services to support their transition and settlement into Canada. The Settlement Health Navigator will deliver specialized social work services to support newcomers to navigate and access healthcare and mental health care, and disability supports.. This role will also work to create an environment that is accepting and welcoming of diversity.




Roles and Responsibilities:



  • Conduct comprehensive client assessments on the overall health, abilities, and needs of the clients (includes specialized supports for newcomers with disabilities) 

  • Facilitate continuity of care by providing timely and appropriate referrals or connections for clients to access necessary medical services, with the goal of  improved health outcomes

  • Connect clients to the full suite of healthcare resources in their resettlement community and reduce unnecessary reliance on emergency services and departments

  • Follow up with health professionals to ensure treatment plans and referrals are scheduled and attended by clients

  • Guide and educate clients on navigating the Provincial Health Care system, ensuring understanding and engagement with the process

  • Facilitate access to specialized services, accommodations, and resources that cater to the specific needs of individuals with disabilities

  • Perform health-related social work with individuals/families, and in group settings in office, school and home settings

  • Perform crisis intervention as well as solutions-focused guidance to newcomers experiencing difficulties adjusting

  • Deliver the Refugee Health Screener 15 (RHS-15) a tool for screening refugees for emotional distress and mental health, to aid in urgent referrals 

  • Work collaboratively with ANC staff to support the healthcare needs of medically complex clients, as well as the wider GAR population 

  • Participate in and collaborate with health-related committees, maintaining and supporting partnerships with Public Health, NL Health Services, private clinics, and specialists

  • Develop and perform presentations for clients, staff, and community partners

  • Create a positive, respectful environment for clients, addressing their immediate needs and supporting their overall well-being



  • Ensure confidentiality of client information in all aspects of service delivery

  • Ensure program activities are delivered in a manner consistent with a trauma informed approach

  • Record and compile client profile and service statistics using NewOrg, to include in monthly and quarterly iCARE activity reports and annual narrative reports


Qualifications and Experience:



  • Bachelors of Social Work and must be a registered Social Worker (or eligible for registration) (required)

  • Experience working with vulnerable populations, preferably in a case management capacity

  • Cultural awareness and sensitivity in working with diverse populations

  • Demonstrated ability to manage clients with complex medical, psychosocial, mental health, or addiction challenges

  • Proficiency in Microsoft Office Suite and Google Workspace

  • Strong communication skills and ability to work in a team-oriented environment

  • Ability to prioritize referrals, manage multiple tasks, and maintain attention to detail

  • Crisis intervention and conflict resolution skills

  • Knowledge of child welfare and community development services

  • Competence in case file management and preparing stakeholder reports


Conditions of Employment:



  • Legal authorization to work in Canada 

  • Flexible to work outside of core hours (Monday-Friday, 9:00am - 5:00pm) occasional evenings or weekends may be required based on client needs.

  • Valid class 5 NL driver’s license and daily access to a reliable vehicle is required

  • Some travel may be required

  • Position is subject to a satisfactory Criminal Record Screening and Vulnerable Sector Check from the Royal Newfoundland Constabulary
