Position Summary:
In this role, you will provide compassionate care to residents with chronic conditions, assisting with daily activities, administering medications, and managing treatment plans.
- Provide direct patient care following care plans.
- Administer medications and treatments.
- Monitor and report patient condition changes.
- Assist with daily living activities (ADLs).
- Perform wound care and specialized procedures.
- Collaborate with the healthcare team.
- Document patient progress accurately.
- Educate patients and families on care.
- Ensure infection control and safety compliance.
- Deliver compassionate, patient-centered care.
