SOCIAL WORKER


(TEMPORARY MODIFIED FULL TIME (0.8 FTE))


 


A temporary modified full time (0.8 FTE) (Tuesday-Friday) opportunity is available at our Louise Marshall Hospital site of North Wellington Health Care.


 


This position will be required to provide back-up to the Palmerston and District Hospital, site of North Wellington Health Care. Additionally, travel is required to Groves Memorial Community Hospital to support the Diabetes Program, as scheduled, one day per week. 


 


Working collaboratively as an integral member of the interdisciplinary team, the Social Worker plays a critical role in providing social work services for elder life care, diabetes patients and their families, discharge planning, psychosocial and/or family needs. There is a strong emphasis on psychosocial support, interdisciplinary problem solving and liaising with appropriate members of the team to ensure patients and families are linked with necessary resources and supports. The Social Worker will be able to prioritize a very diverse caseload of patients and respond to crisis situations as they arise.  The Social Worker will promote continuity of care as patients are transitioned from hospital to home with OHaH, Community Partners, and the Family Health Teams.

RESPONSIBILITIES:


The Social Worker will demonstrate exceptional leadership skills, a high level of professional integrity and accountability and play a key role in system development, as well as possess a strong level of understanding of care across the continuum.



The Social Worker will provide support in three key areas: Patient Support/Discharge Planning, Diabetes Education, and Volunteer Visitor Program. 


 


Patient Support/Discharge Planning and General Social Work: 



  • Assessing patients for the purpose of evaluating the need for service, the nature of the problem, formulating recommendations or treatment plans and implementing intervention plans 

  • Supportive care of patients and families with new and/or recurrent diagnoses 

  • Coordination of discharge planning with OHaH and Community Supports 

  • Responding to the Emergency Department to provide crisis support for emergency and/or urgent situations 

  • Assessing risk and making appropriate referrals as needed – Seniors at Risk, Family and Children’s Services etc. 


 


Diabetes Education – Working closely with Diabetes Nurse Educators and Registered Dietitians, the Social Worker’s focus of service delivery will be to integrate therapeutic practice for individuals and families living with diabetes and chronic disease.



Patient Visitor Program - the Social Worker will recruit, train, and oversee volunteers in the Patient Visitor Program, which provides therapeutic visits and meal assistance to inpatients at the hospital. 



QUALIFICATIONS:



  • University graduate from a recognized academic program of Social Work with a Bachelors of Social Work required; MSW preferred

  • Registration and membership in good standing with the Ontario College of Certified Social Workers and Social Services Workers

  • Experience in short-term supportive counselling with adults with diabetes patients and their families would be a valuable asset

  • Recent working experience in a hospital setting with the elderly population, preferably in an inpatient setting

  • Experience working with volunteers with the ability to provide leadership and mentorship

  • Excellent patient assessment and interviewing skills

  • Understanding of loss/adjustment and the grief process

  • Strong advocacy skills 

  • Demonstrated strengths in interpersonal relations, problem solving, and conflict resolution

  • Demonstrated commitment to a patient/family centered care environment

  • Demonstrated excellent critical thinking and problem solving skills

  • Demonstrated ability to work effectively within an interprofessional team as well as independently while initiating, prioritizing and managing multiple tasks

  • Evidence of ongoing professional development, continuing education and leadership experience

  • Proficient computer skills, knowledge of Meditech system is of benefit



  • Outstanding organizational skills, attention to detail, and ability to keep careful documentation

  • Excellent communication skills (both written and verbal), interpersonal, and public speaking skills

  • Demonstrated good attendance and work record with the ability to maintain this same standard

  • Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

  • Satisfactory performance and attendance required

  • Satisfactory Reference Checks required prior to start date

  • Must provide a Police Vulnerable Sector Check

  • Must complete a Pre-Employment Health Assessment prior to being hired 


 


SALARY: $44.67 - $57.90 per hour
