Program:
Lotus Group Care is a resource providing 24-hour care and protection for youth, during periods of crisis and transition on a long-term treatment basis. This program targets youth 12 years of age and under. The program consists of in-home and out-of-home programming. Programming is designed to support youth who may be struggling with mild to moderate behavioral or emotional health during periods of crisis or transition. The typical duration of stay for the clients is based off of each individual child’s need, potentially short or long term care.
Role:
The role of the Support Liaison is to develop and implement both group and individualized programming initiatives for persons-served. The Support Liaison role is dynamic, and requires flexibility, priority management skills, and the ability to work collaboratively with multiple teams. The Support Liaison must anticipate and respond to both operational and service delivery needs of the program.
The Support Liaison will report directly to their Program Manager and indirectly to their division’s Service Enhancement Coordinator (Executive Leadership). The Support Liaison will work in collaboration with their home program team as well as the Support Liaison team to create and facilitate initiatives that will drive and inspire personal growth and development for persons-served. Using both trauma-informed and person-centered approaches, the Support Liaison will build meaningful relationships with persons-served and will assist in teaching life and independent living skills to the individuals served in a cooperative environment.
This position includes variable hours and shifts, including but not limited to potential day shifts, awake night shifts, and sleep shifts. The primary hours will be shift will be 5 days per week between noon to 8pm to help support with programming and youth activities. In addition to this, Support Liaisons will be responsible for one (1) weekend on-call per month in their designated home program.
