Transition & Follow Up Worker 



What We Do 


 


YESS is a not-for-profit agency working with youth facing difficult realities. Based in Edmonton, our Youth Support Centre provides immediate and low-barrier 24/7 sleep support and individualized wrap-around supports for young people aged 15 -24. 


 


We work collaboratively within a network of care, focused on the prevention of youth homelessness by providing youth with the necessary supports to stabilize their housing, improve their wellbeing, build life skills, connect with community, and avoid re-entry into homelessness.

What You Will Do 


 


The Transition & Follow-Up Support Worker provides individualized, person-centered and trauma-informed support to youth and young adults as they transition from crisis toward stability and independence. This role focuses on housing stabilization, financial empowerment, employment readiness, and ongoing follow-up support to ensure long-term success. 


 


The worker builds strong, trust-based relationships and supports participants in navigating systems, accessing resources, and developing practical life skills necessary for sustained independence, while maintaining progress in housing, employment, and overall well-being. 


 


Location: 9310-82 Avenue, Edmonton AB 


 


Key Responsibilities 


Case Management & Transition Planning 



  • Manage a caseload of youth and develop individualized service plans  



  • Conduct needs assessments and goal planning  



  • Provide consistent follow-up through regular check-ins and progress reviews 



  • Monitor outcomes and adjust support plans as needed 



  • Maintain accurate case notes, reports, and service documentation  



  • Collaborate with internal and external partners to provide coordinated care  


 


Housing Stabilization 



  • Support youth in securing and maintaining housing, including:  





    • Housing search, applications, and placement  







    • Referrals to appropriate agencies, housing programs and partners  







    • Provide landlord navigation and advocacy:  







    • Advocacy, mediation, conflict resolution, and support with tenancy-related issue 







    • Education and facilitation on tenant rights and responsibilities skills such as Rent Smart 





  • Support housing setup:  





    • Coordinate access to furniture, household items, and basic needs through partnerships  







    • Pantry and food hamper selection  







    • Clothing and personal items  




 


Financial Empowerment & System Navigation 



  • Support financial stability through:  





    • Tax filing assistance and Benefits Navigation  







    • Facilitation or Connection to Empower U and financial literacy programming  







    • Assist youth in obtaining and maintaining ID and essential documentation  







    • Support budgeting, financial planning, and income stabilization  




 


Employment & Skill Development 



  • Support job readiness, including:  





    • Resume building and interview preparation  







    • Job search and applications  







    • Connection to training, certifications, and volunteering for employment qualifications and resume improvement  







    • Develop business partnerships to support applications 





  • Facilitate life skills workshops such as:  





    • Social and Independent living skills  




 


Follow-Up & Case Management 



  • Provide ongoing follow-up support post-transition:  



  • Regular check-ins (in-person, phone, etc.)  



  • Monitor and document progress toward goals (housing, employment, financial stability)  



  • Develop individualized support plans with clear goals and outcomes  



  • Maintain accurate documentation and case notes  



  • Collaborate with internal teams and external partners to ensure coordinated care  



  • Ensure youth are connected to internal and external supports that promote stability and well-being

Qualifications & Experience 



  • Experience working with individuals experiencing:  





    • Homelessness  







    • Trauma  







    • Mental health and/or addiction challenges  





  • Strong knowledge of:  





    • Housing systems and landlord-tenant processes  







    • Income supports and community resources  







    • Employment and life skills development  





  • Experience facilitating group sessions, workshops, or educational programming

  • Demonstrated ability to build, develop, and maintain strong external partnerships

  • Diploma or degree in Social Work, Human Services, or related field (or equivalent experience) is an asset 

  • Experience in financial empowerment, benefits navigation, or tax filing is an asset  


 


What You Bring 



  • Valid First Aid Certificate completed within the last twelve months.  



  • Suicide intervention, harm reduction, and crisis intervention training is considered an asset.  



  • Clear police clearance (PIC) and a Child Intervention (CIRC) check, both obtained within the last six months 



  • Valid Alberta class 5 or higher driver’s license and clean driver’s abstract  



  • $1 million PLPD Personal Liability Protection
