THE OPPORTUNITY

We are looking for Wellness and Rehab Practitioner to join our The Gardens at Qualicum Beach team in Qualicum Beach, B.C. 

Summary of Duties & Responsibilities:
The Wellness and Rehabilitation Practitioner drives for the best possible physical wellness for our residents. The ideal candidate works with/helps occupational therapists, physical therapists, recreational therapists, and speech -language pathologists to promote, restore, maintain, and /or enhance a client’s level of functioning in the areas of mobility, self-care, productivity, leisure, and day-to day activities.
 

Duties and Responsibilities:

• Carry out rehabilitative therapeutic interventions.
• Carry out group exercise programming to promote wellbeing.
• Help in the modification of care plans.
• Create and facilitate programs to assist clients with complex disabilities in daily living to attain their goals to maximize potential for independence.
• Report relevant observations of a client’s condition and document changes.
• Help manage equipment safety and inventory.
• Assist in the smooth transition of resident admissions, discharges, and transfers.
• Resident care planning.
• Working with electronic documentation (PointClickCare)
• Liaises with physicians, families, and other members of the care team.
• Prepares recreation reports for medical team and care conferences.
• Assist in improving and maintaining the overall wellbeing of our residents.
• To maintain the walking Program under the direction of the Physiotherapist/Occupational Therapist
• To implement the daily group exercise program created with the Physiotherapist and Occupational Therapist with input from the Physiotherapist and Occupational therapist.
• To assist in the planning and implementation of a small group activity in partnership with the Recreation team.
• To develop and maintain “getting to know you” information with all residents.
• Assist in the development and implementation of care plans from an activity perspective.
• Documentation in Point Click Care progress notes.
• Maintain attendance records for activities.               
• Other duties as required

Qualifications:

• Rehab diploma (2-year program) from a recognize school or equivalent and/or experience working (2 years) as a Rehab/Therapy Assistant

• Work experience varied – New Graduates welcome.
• Must possess the personal qualities and attitudes that respect and maintain the spirit, dignity, and individuality of the residents.
• Must have a commitment to excellence in Care of the older person in a Long-Term Care environment.
• Must be committed and understand working with residents with dementia.
• Must have excellent communication skills and interpersonal skills and is able to establish and maintain relationships with Ownership/management team, staff, residents, resident’s families, and external agencies.
• Ability to work independently but also guide and lead a team of staff members.
• Demonstrates good time management.
• Has a commitment to ongoing professional development for self and others.
• Physically and mentally able to perform duties for above position.
• Be a role model for other staff members.
• Show a professional, caring, and considerate attitude to residents, visitors, and staff.
