Job Description - Youth and Family Advocate

Tla’amin Health operates within the Tla’amin Nation. The Tla’amin Nation is a modern, forward thinking, self-governing nation located on the beautiful upper Sunshine Coast (qathet Region).


Position Summary
Job Status:        Permanent Full Time
Job Title:            Youth and Family Advocate

Working with the Tla’amin Health team across programs and services at Tla’amin Health reporting to the Health Director. The Youth and Family Advocate (YFA) works with Tla’amin youth and families to continue to keep their children healthy, well and safe. 


The successful applicant will be required to work in accordance with a BCGEU Collective Agreement as well as the policies and benefits therein and addition to all relevant Tla’amin Health policies and procedures. This position is open to applicants of all genders and requires union membership


Key Responsibilities 
Performs all duties and responsibilities in accordance with Tla’amin policies, standards, practices and procedures as directed by the Director of Health Services or designate. Maintains confidentiality as required pertaining to members, clients, employees and general matters of the Nation. 


Roles and Responsibilities


1)    Family Health and Wellness Management
•    Develop positive and healing relationships with the Tla’amin community and its families by always centering dignity.
•    Work with families to reconnect with healthy ways of being such as developing parenting, self-empowerment, and
       relationship skills.
•    Coordinate with other Tla’amin Health and Nation Children and Family Services staff to ensure wrap-round services for
      all Tla’amin youth and families.
•    Provide goal-oriented support rooted in traditional teachings that strengthen access and commitment to health within the
      home.
•    Advocate for clients within Health, Education and other health care systems.
•    Build relationships with service providers to facilitate a positive experience for families.
•    Keep up-to-date, clear, and confidential records to the standard of Tla’amin Health.
•    Attend family emergencies and outreach, as appropriate and as required in collaboration with the Child and Family
      Services team.
•    Attend committees and other meetings related to job duties and role.
•    Actively engaging to build relationships with service providers to facilitate a positive experience for families.
•    Effectively navigate Electronic Medical Records (EMR) with a focus on managing reports, meetings, and advocacy.


2)    Children and Youth with Special Needs - Complex behaviour key worker:


•    Understand existing programs, policies, and services, supporting families in accessing services, advocacy, building
      relationships among parents and service providers, sharing information in an accessible way with community,
      relationship-building, EMR and report writing.


3)    Teamwork:
•    Communicate regularly with the Case Manager/Navigator and Child and Family Services team.



Qualifications
•    Bachelor’s degree in child and Youth Care, Social Work, or a related field combined with 3-5 years of directly related              experience, or an equivalent amount of education and experience will be considered.
•    Leadership training and/or experience
•    Extensive knowledge of Tla’amin culture, community, and language, as well as guiding strategic plans (ie. Tla’amin
      Nation Health and Wellness Plan; Comprehensive Nation Plan) an asset.
•    Demonstrated capacity to practice in ways that are culturally safe, and trauma informed.
•    Excellent verbal and written communication skills.
•    Extensive understanding of FOIPPA, confidentiality, and related responsibilities.
•    Experience in providing advocacy within Child Protection, Health, Education, Justice and other systems for Indigenous
      people and/or communities.
•    Ability to organize and manage multiple resources, groups, and community data within the digital case management                systems.
•    Ability to be independent, motivated and innovative
•    Demonstrated ability to work collaboratively with multi-disciplinary service partners and stakeholders
•    Advanced computer skills especially Microsoft Office: Word, Excel, Outlook & PowerPoint
•    Mental Health First Aid, Basic First Aid, Naloxone Training, Nonviolent crisis intervention training, and related                            certifications are an asset
•    Understanding of mental and emotional wellness, and cultural safety
•    Class 5 B.C. Driver’s License and acceptable Driver’s Abstract
•    Acceptable Criminal Record Check – Vulnerable Sector and acceptable history check related to child protection (MCFD)        before employment.



Working Conditions
Work is performed primarily in an office environment at Tla’amin Health. Occasional travel to attend meetings or training may be required.


Additional Information
All employees working for Tla’amin Health must be fully vaccinated for COVID-19. Proof of vaccination status will be required.
