Job Description - Aspen Community Services Support Worker (3pm to 9 pm) 7 on 7 off (Internal Candidates only)

Reporting to the Program Manager and the Caseworker is responsible for and provide quality case management of the individuals, within the Ministry of Children, Community and Social Services guidelines.


The Direct Support Professional provide comprehensive personal support to individuals experiencing life with Developmental Disabilities and secondary disorders. Support is provided in office/center, in home, in the community and virtually with individuals in the Greater Sudbury Area.


The DSP provide professional support services and assesses the individual’s readiness for change in any of the areas of living, learning, working and social development. The Caseworker and DSP work directly with the individuals to support them to successfully transition to live with host families, with natural families, and independently, and to participate fully in their community. Staff work collaboratively with host family providers, natural families, external agencies, community groups, and co-workers in CLGS programs.



  • Work closely with adults living with a developmental disability and additional disabilities

  • Meet with individuals, either in their home, in the center, over the phone or in the community

  • Communicate with and care for several individuals in a group setting, alone and with one or more other staff

  • Facilitate community involvement whether they are medical, social in nature, recreational or vocational

  • Assist individuals in accessing and obtaining other community resources where needed and appropriate and advocate with and for individuals to ensure adequate resources are available

  • Remain with individuals on the community while assisting the individual to utilize public resources and maneuver through their community (i.e., public transportation)

  • Where resources do not exist or are inadequate, advocate within the system to develop or improve essential services and resources for individuals with disabilities and mental health illnesses

  • Treat each person as an individual and encourage the personal and independent development of self. Consider the choices, needs, and wishes of each person involving them in their own plans and day-to-day decisions

  • Staff are responsible for building and maintaining positive and professional relationships with the individuals’ guardians / family, host family, health professionals, community organizations and other professional agencies


Travel is required for this position. Only candidates with G class drivers' license and access to a reliable vehicle will be considered. Out of town travel may be necessary on occasion.


This position will be for a 7 days on 7 days off shift rotation.
