We serve a remarkable community of individuals with diverse backgrounds and experiences. Since founding in 1987 as part of the Welcome Home program, we've been dedicated to supporting adults with intellectual disabilities as they transition from institutional settings to vibrant community living. We provide 24-hour support to individuals with Autism Spectrum Disorder, FASD, Mental Health challenges, problematic substance use, and behavioural and sensory challenges, ensuring each person receives the care, respect, and opportunities they deserve. We're committed to empowering individuals to thrive as valued members of their communities, celebrating their resilience and unique journeys every day.    



Cluster Outreach Support Mentor Job Summary: 


As a Cluster Outreach Support Mentor, you will provide individualized support to individuals both within the cluster location and in the community, promoting independence, safety, and meaningful engagement in daily life. You will work closely with multiple individuals throughout the shifts, assisting with occasional personal care, life skills, emotional wellness, and community access. 


This particular opportunity is to support individuals residing in 8 suites within a 77-suite building. This is a unique opportunity for professionals who excel at building relationships, taking initiative, and working independently while maintaining strong professional standards. 


The ideal candidate would have Crisis Intervention experience, work well under pressure, have strong boundaries and the ability to de-escalate conflicts. Experience with mental health is an asset. 



Preference may be given to candidates with experience or training in: 



  • Non-Violent Crisis Intervention (NVCI)  



  • Security experience  



  • Shelter experience  



  • Naloxone training  



  • Suicide Prevention / ASIST training 



Available Shifts: 


We are currently hiring for the following schedules, with coverage required 7 days per week: 



  • 6:00 PM – 6:00 AM (Awake Overnight – male staff preferred)  



  • 12:00 PM – 6:00 PM (female staff preferred)  



Key Responsibilities 



  • Provide one-on-one and cluster-based support to individuals in cluster home and community settings  



  • Support individuals with meal planning, grocery shopping, cooking, and household upkeep  



  • Administer and document medication regimens according to protocol  



  • Attend and advocate during medical appointments  



  • Provide emotional support, companionship, and crisis intervention  



  • Assist with life skills development, including budgeting, bill payments, transportation, and self-care  



  • Support individuals in accessing employment, volunteer roles, programming, and recreational opportunities  



  • Facilitate connections to community resources such as food banks, clothing resources, and support agencies  



  • Encourage social inclusion and participation in community activities  



  • Maintain clear, accurate, and timely documentation  



  • Build strong, professional relationships while maintaining healthy boundaries  



  • Work independently and collaboratively with team members to meet support goals  


 


Qualifications 



  • High School Diploma (minimum)  



  • 1–2 years of experience working with adults with intellectual disabilities, transferable life skills, and relevant lived or professional experience will also be considered 



  • Some post-secondary courses in the social services field (preferred) 



  • Knowledge of disabilities, disorders, and mental health issues 



  • Strong communication skills  



  • Advocacyflexibility, and the ability to adapt to changing support needs 



  • Comfortable working alone




Required Certifications & Requirements 



  • Current First Aid & CPR Certification  



  • Valid Manitoba Driver’s License  



  • Access to a safe vehicle for work purposes  



  • Criminal Record Check with Vulnerable Sector Search  



  • Adult Abuse Registry Check  



  • Child Abuse Registry Check
