The Outreach Registered Nurse (RN) is a key member of the Guelph CHC Outreach Nursing team, working alongside Guelph-Wellington HART Hub and the Integrated Programs of the CHC. This role provides trauma-informed, harm reduction–centered, and palliative-informed nursing care to individuals with life limiting illness who are homeless or precariously housed. This role has a Palliative focus.

Reporting to the supervisor, The Outreach RN will provide care, support, and education to individuals in the community, shelters, drop-ins, encampments, parks, and other outreach settings. The RN collaborates closely with Nurse Practitioners, physicians, allied health, community paramedicine, mental health and addictions services, shelters, hospice/palliative care providers, and other community agencies to provide wraparound, coordinated care. This includes supporting care transitions between the community, emergency department, and acute care settings. ins, encampments, parks, and other outreach settings. 

Core Responsibilities:

• Deliver nursing care across in community, and mobile outreach settings, with a strong focus on supporting individuals with life limiting illnesses through palliative and comfort-focused interventions. 


• Provide comprehensive palliative nursing assessments, symptom management, and comfort-oriented care in collaboration with NP/MD and the interdisciplinary team.


• Administer medications, injections, and treatments—including pain and symptom management therapies—and provide skilled wound care tailored to palliative needs.  


• Support coordinated care planning and advocate for clients to reduce barriers and ensure equitable access to palliative and community services.  


• Provide crisis intervention, code response, and deescalation as needed, maintaining client safety and emotional wellbeing. escalation as needed, maintaining client safety and emotional wellbeing.

Required Qualifications: 

• Baccalaureate in Nursing. 

• Current Certificate of Competence with the College of Nurses of Ontario (RN). 

• Emergency First Aid (or equivalent) and CPR Level C. 

• Demonstrated experience working in a manner consistent with the principles of harm reduction, trauma-informed, and palliative nursing care 

• Strong understanding of palliative approaches. 


• Ability to build rapport with vulnerable populations. 


• Passion for providing care to individuals who use substances. 

• Ability to work under pressure and independently. 

• Knowledge of internal/external services in Guelph and Wellington County. 

• Strong assessment and care planning skills.

• Strong communication skills. 

• Ability to maintain professional boundaries and confidentiality. 

• Assets: LEAP, naloxone training, OAT Fundamentals, wound care, NVCI, motivational interviewing. 

Working Environment: 

• Weekend/evening work may be required. 

• Combination of desk/office work, with majority of time spent in community. 


• Work involves interaction with individuals experiencing mental health challenges, addictions, homelessness, and justice involvement.
