Job Description - Part-time Social Worker or Occupational Therapist

The Centre for Family Medicine (CFFM) Family Health Team is seeking a


Part-time Social Worker or Occupational Therapist (8-16 hours per week) for the


CFFM and Regional MINT Memory Clinics



The CFFM’s mission is “To provide leadership and excellence in comprehensive patient-centred primary care through education, research, innovation and care provision in a collaborative, sustainable, inter-professional environment.”


 


The Centre for Family Medicine's academic Family Health Team is comprised of four sites and is an integral part of the University of Waterloo Integrated Health Building as well as the McMaster Waterloo Regional Medical Campus. We work in a fully computerized environment and encourage continued educational experiences.


 


MINT Memory Clinics


The Multi-specialty INterprofessional Team (MINT) Memory Clinic is an inter-professional approach to providing early diagnosis and treatment for persons living with memory loss. Team members work with family physicians to provide comprehensive care for patients and family members dealing with dementia and other conditions involving memory loss. Our goal is to improve access to high quality person-centered dementia care for older adults and their caregivers through team-based care provided in primary care.


 


Position Overview


Reporting to the Elder Health Manager, this position is a key in-clinic presence supporting clinic operations and assessment across both CFFM MINT Memory Clinic and Regional MINT Memory Clinic in Kitchener-Waterloo (KW).



Responsibilities


In-Clinic



  1. Work within an inter-disciplinary team to provide a comprehensive assessment for memory loss concerns. Conduct thorough collateral assessments with individuals who are care partners to persons presenting with memory loss, including administration of outcome measures and scales.

  2. Communicate with patients, care partners and other staff to provide a high-quality care experience.

  3. Respond to referral source, memory clinic team, and patient/care partner inquiries as needed.

  4. Provide detailed, timely, and appropriate documentation for clinical activities.

  5. Participate in clinic and program evaluation as required.


 


Patient and Family Coordination



  1. Conduct assessments, prepare care and/or service plans, provide case management and make appropriate service referrals when required as part of the interprofessional memory clinic team.

  2. Provide crisis management and follow up plans as required, including liaising with patients and/or family or other care providers.

  3. Maintain accurate records and progress notes in the care plan.

  4. Address patient/family clinical issues as they arise and liaise with appropriate team members to support patient/family inquiry.

  5. Participate in ongoing quality improvement and continuing education initiatives.



Additional tasks as required.



Requirements



  • Registered Social Worker or Occupational Therapist in good standing with the respective provincial college

  • Experience working with individuals with memory loss an asset

  • Experience working in inter-disciplinary teams and liaising with clinicians, patients, families and other care givers

  • Experience with records management in a healthcare environment

  • Strong analytical and problem-solving skills

  • Ability to work with ambiguity and minimal supervision

  • Excellent written and verbal communication skills

  • Solid organizational skills including attention to detail and multitasking skills
