Our Community Homes for Opportunity (CHO) program lives this ideal by providing mental health and addictions supports to CHO homes and encouraging residents to become active members of their community. As a member of the CHO team, you'll work with residents as well as independent CHO home staff to enhance the quality of life for each individual resident and to empower the ability to gain life skills, grow their personal strengths, and gain choices for their wellbeing.

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role for RPNs is $30.01 per hour (Level 1 – Direct Service, Regulated), or for RNs, is $32.13 per hour (Level 1 - RN/OT)
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $37.26 per hour for RPN's and $41.41 per hour fir RN's
    • For internal applicants, grid level placement will occur as per the Collective Agreement
  • When you'll work - Shifts in CHO are between Monday to Friday, 8:00am - 5:15pm
  • Your pension plan - Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)


HOW YOU WILL BE MAKING A DIFFERENCE:

Nursing Assessments 
  • Conduct physical and psychiatric assessments and make appropriate referrals to community physicians and resources
  • Assess and document medical effectiveness, complications, and side effects
  • Identify, develop, and maintain strong links with a wide range of community partners/resources
  • Organize, coordinate, and administer depot injections
  • Review and audit completion of Medication Administration Records (MARs) completed by staff within the home, completing incident reports and taking appropriate measures in response to medication errors to ensure health and safety of affected tenants
Health Promotion
  • Assist clients in making informed decisions about health measures such as medications, immunization, palliative care, disease management, etc. 
  • Provide health teaching by giving feedback, explanations, and rationale for diagnostic tests, procedures, and treatments
  • Support education and medical management policy and procedure training of CHO private home staff
  • Assist residents in taking responsibility for and managing their health needs according to available resources and personal skills
  • Identify and seek to address the root causes of illness and disease
  • Assist residents in making healthier choices
  • Advocate for residents to obtain psychiatrists, family doctors, dentists, etc.

Crisis intervention:

  • Identify high risk residents and assess the need for crisis intervention, prioritizing the safety of residents
  • Utilize harm reduction principles to identify, reduce, and remove risk factors
  • Respond to urgent health concerns


WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

We require the following qualifications:

  • Diploma or Degree in Nursing as a Registered Practical Nurse or Registered Nurse (Primary proof of credentials will be required during the hiring process (i.e. original degree/diploma/certificate or official transcript). For ongoing education, an unofficial transcript will be accepted)
  • Current registration from the College of Nurses of Ontario in good standing
  • Valid Driver’s Licence; vehicle access and auto insurance with at least $2 million in liability insurance for transporting clients. 
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:

  • Certification in ASIST and First Aid/CPR
  • Bilingual in both official languages, preferred; préférence sera accordée aux candidats qui sont compétents dans les deux langues officielles du Canada
