Job Description - Supported Attachment/Virtual Float Nurse Practitioner


Position:  Supported Attachment/Virtual Float Nurse Practitioner


Employment Type:  Part-Time Contract until March 31, 2027 (possible extension pending funding)


Vacancy Status:  New Position


Salary Range:  $62.67-64.55 per hour


Location:  Hybrid

We provide accessible, quality, culturally safe and holistic health care that supports and enhances the wellness of individuals, families, and communities. We work with our 11 First Nation community partners and the IFC in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services, and health promotion services.


 


The opportunity: As a key member of Maamwesying North Shore Community Health Services Inc., the Supported Attachment / Virtual Float Nurse Practitioner (NP) is an integral part of an interdisciplinary health care team.  This NP role combines two core responsibilities:


1.  Supported Attachment NP – facilitates patient onboarding to primary care, providing clinical care, orientation to the health system, and preparation for long-term attachment to a primary care provider.


2.  Virtual Float NP – provides flexible, remote coverage across sites to ensure timely access to care during provider absences.


The NP ensures patients receive timely access to primary care, supports continuity of care, and strengthens patient-provider relationships, both virtually and in-person, across all Maamwesying North Shore Community Health Services Inc. sites.


 


What you need to bring with you:



  • Master’s in nursing degree with valid Nurse Practitioner certification, preferably in Primary Health Care.

  • Bachelor of Science in Nursing degree.

  • Current registration in good standing as a Nurse Practitioner with the College of Nurses of Ontario (CNO).

  • Minimum 2 years’ experience in community health or primary care preferred.

  • Experience providing virtual care considered a strong asset.

  • Demonstrated ability to work independently and collaboratively in an interprofessional, remote, and in-person environment.

  • Knowledge of Indigenous health issues and commitment to culturally safe care is considered a strong asset.

  • Skills and Abilities

  • Strong critical thinking and clinical decision-making skills.

  • Accountability, dependability, and reliability.

  • Culturally and politically sensitive.

  • Ability to work independently, as well as part of an interprofessional team.

What to expect?


Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur.
