Job Description - Case Coordinator

Assess client needs and develop care/support plans

Coordinate with professionals (doctors, therapists, teachers, social workers)

Schedule appointments and services

Monitor client progress and update records

Act as a communication link between clients, families, and service providers

Ensure services are delivered efficiently and on time

Maintain documentation and reports

Help clients access community resources and support systems

Requirements

Knowledge of case management and care coordination
Strong organizational and communication skills
Problem -solving and decision -making ability
Empathy and ability to handle sensitive situations
Basic computer and documentation skills
