Job Description - Case Coordinator

Coordinate patient care plans and treatment schedules.

Act as a liaison between patients, doctors, nurses, and other healthcare providers.

Schedule appointments, investigations, and follow -up visits.

Maintain accurate patient records and documentation.

Monitor patient progress and report concerns to the healthcare team.

Educate patients and families regarding treatment plans and available services.

Coordinate referrals and discharge planning.

Ensure compliance with organizational policies and healthcare regulations.

Assist in resolving patient concerns and service -related issues.

Prepare reports and maintain case management data.

Requirements

Bachelor's Degree in Healthcare Administration, Social Work, Nursing, Psychology, or a related field.

1–3 years of experience in healthcare coordination, patient services, or case management.

Strong communication and interpersonal skills.

Good organizational and time -management abilities.

Proficiency in MS Office and healthcare information systems.

Ability to handle confidential patient information.
