Job Description:

Job Title: Community -Based Rehabilitation (CBR)
Worker / Coordinator

Location: Anekal

Reporting to: Project Coordinator


Job Purpose

To implement community -based rehabilitation (CBR) activities
for people with disabilities (PwDs) using a rights -based and inclusive
approach. The role involves identifying and supporting people with disabilities
at the community level, facilitating access to rehabilitation services,
promoting inclusive education, and strengthening community structures such as
Disabled People’s Organisations (DPOs) and sub -centres.


Key Responsibilities

1. Identification, Screening and Assessment

  • Conduct
    community -level surveys and door -to -door visits to identify people with
    disabilities.
  • Organise
    and conduct screening camps in collaboration with health, education and
    social welfare departments.
  • Support
    functional, physical, sensory and psychosocial assessments of children and
    adults with disabilities.
  • Maintain
    updated case records and databases of all identified PwDs in the project
    area.

2. Rehabilitation Services and Follow -Up

  • Develop
    individual rehabilitation plans (IRPs) in consultation with the
    multidisciplinary team (therapists, doctors, special educators,
    counsellors, etc.).
  • Provide
    basic home -based rehabilitation exercises and training to PwDs and
    caregivers as per guidance from professionals.
  • Facilitate
    referrals to hospitals, therapy centres, government departments and other
    service providers.
  • Ensure
    regular follow -up, monitor progress, and document outcomes of
    rehabilitation interventions.

3. Community Structures: DPOs and Sub -Centres

  • Facilitate
    the formation and strengthening of Disabled People’s Organisations (DPOs)
    and self -help groups of PwDs and caregivers.
  • Support
    community sub -centres/rehab corners as outreach points for therapy,
    counselling, assistive devices and information.
  • Build
    leadership capacities of DPO members to advocate for their rights and
    entitlements.
  • Organise
    regular meetings, planning sessions and review meetings with DPOs and
    community volunteers.

4. Inclusive Education

  • Identify
    children with disabilities who are out of school or irregular in school.
  • Coordinate
    with schools, ICDS/Anganwadi centres, special schools and resource centres
    to promote inclusive education.
  • Support
    school enrolment, retention and learning of children with disabilities
    through accommodations and referrals.
  • Sensitise
    teachers, School Management Committees (SMCs), Anganwadi workers and peers
    on disability inclusion.
  • Facilitate
    provision and use of assistive devices, learning aids and accessible
    learning materials.

5. Community Awareness and Social Inclusion

  • Organise
    awareness programmes, community meetings, street plays, rallies and
    campaigns on disability rights and inclusion.
  • Sensitise
    community leaders, local governance bodies (Panchayats/ULBs), self -help
    groups and other stakeholders.
  • Promote
    participation of PwDs in local development programmes, livelihood schemes
    and social protection schemes.

6. Coordination, Networking and Linkages

  • Coordinate
    with government departments (health, education, social welfare, women and
    child, rural development, etc.) for convergence and benefits.
  • Facilitate
    PwDs’ access to disability certificates, ID cards, pensions, scholarships
    and other schemes.
  • Maintain
    regular communication with partner organisations, community volunteers and
    service providers.

7. . Livelihood and Social Inclusion

  • Identify youth and adults with disabilities who are interested in livelihood
    opportunities.
  • Facilitate
    access to skill development, vocational training, self -employment
    schemes, and wage employment
    through government and private partners.
  • Support
    formation of self -help groups (SHGs), producer groups or collectives involving persons with disabilities and caregivers.
  • Link
    persons with disabilities to social security schemes, pensions,
    health insurance, and other welfare programmes.
  • Promote participation in community events, local governance (Gram Sabha), and
    citizen forums
    to ensure inclusion and voice.

 

8. Documentation, Reporting and Monitoring

  • Maintain
    case files, registers and MIS for all project activities and
    beneficiaries.
  • Prepare
    monthly, quarterly and annual activity reports and share with the
    supervisor.
  • Document
    good practices, success stories and challenges from the field.
  • Support
    in data collection for baseline, midline and end -line assessments or
    evaluations.

Qualifications and Experience

  • Any
    Degree in in Social Work, Sociology,  Rehabilitation Science, Education or
    related field / CCRT / CCCG
  • Preferable:
    CCRT / CCCG / Diploma/Certificate in Community -Based Rehabilitation /
    Disability Studies / Special Education.
  • 1–3
    years of experience in community work, disability sector, health,
    education or development projects (freshers with strong motivation may
    also be considered).
  • Experience
    working directly with people with disabilities and their families will be
    an added advantage.

Skills and Competencies

  • Strong
    commitment to inclusion, rights of people with disabilities and community
    development.
  • Basic
    knowledge of different types of disabilities and rehabilitation
    approaches.
  • Skills
    in community mobilisation, counselling and group facilitation.
  • Ability
    to communicate clearly with diverse community members, including in local
    language.
  • Good
    documentation and basic computer skills (MS Office, data entry).
  • Ability
    to travel extensively within the project area and work closely with
    communities.
  • Team
    player with problem -solving attitude and cultural sensitivity.

Terms and Conditions

  • Position:
    Full -time
  • Contract:
    [Insert duration, e.g., 1 -year renewable based on performance and funding]
  • Remuneration:
    As per organisational norms and experience.
