Job Description - Community Social Worker

Conduct community outreach and identify individuals/families in need.

Assess beneficiaries' social, emotional, and economic needs.

Develop and implement individual care/support plans.

Facilitate access to healthcare, education, welfare schemes, and community resources.

Provide counselling, guidance, and psychosocial support.

Conduct awareness programs and community education activities.

Coordinate with government departments, NGOs, and local stakeholders.

Maintain beneficiary records, case documentation, and reports.

Monitor beneficiary progress and conduct follow -up visits.

Participate in community development initiatives and advocacy activities.

Ensure confidentiality and uphold professional ethics.

Perform other duties assigned by the reporting officer.
