ROLE
DESCRIPTION:

The Data Manager is responsible to ensure smooth transition through the
continuum of care to achieve clinical, financial and satisfaction outcomes
for case management. Provides resources for all
hospital and clinic patients as needed. The primary responsibilities surround
psychosocial services and discharge planning/transition management functions
for assigned patient population. Coordinate with medical team for
necessary updates for the patients and help bridge/ facilitate a dialogue
between patients and medical team

SKILL SET:

1.      Must
possess computer knowledge like MS office and internet

2.      Good
spoken and written English language

3.      Preferred
specialization in pediatric experience

4.      Hospital
experience

5.      Good
problem-solving skills.

 

KEY RESULT
AREAS:

1.      Coordinate with medical team for necessary
updates for the patients

2.      Help bridge/facilitate a dialogue between
patients and medical team

3.      Recording of all data related to patients
in Salesforce/vCan

4.      Preparing Weekly, Maintaining Monthly and
Quarterly Reports and Data.

5.      Responsible
for handling the unit as assigned.

 

DUTIES
AND RESPONSIBILITIES:

 

Operational:

1.      Responsible for handling all the work assigned
for the Unit that includes:

 

(a)    Going to
Hospital Unit to assist in running of Chattai clinic and coordinate and
implement Cankids support activities as determined for the Center.

(b)    Coordinate
with medical team for necessary updates for the patients and help
bridge/ facilitate a dialogue between patients and medical team 

(c)    Entering
details of child patient into database (Salesforce/vCan) from P.I.S. and
M.S.F and complete all related procedures for the same.

(d)    Form a
bridge between the doctors and the patient family and make sure the child is
given the best care and benefit and spreading the message ‘’you are not Alone
‘’ amongst the families and in the society.

(e)    Update
the database as when a child is adopted or money is donated for his
treatment.

(f)     Timely
& periodically follow up with patients for continuing treatment.

 

 

Data
Management and Reports:

 

(a)    Maintaining
the data of the particular unit assigned.

(b)    Assist in making weekly/ monthly/ quarterly
reports in regular contact with Program Officer (Medical and Hospital
Programs).

 

Administrative:

 

(a)    Ensure proper assistance to pediatric
Cancer unit.

(b)    Spreading awareness about childhood cancer
through awareness and information program.

(c)    Responsible for handling the unit as
assigned

(d)    Responsible for maintaining proper
coordination with the Medical Officer.
