Palliative Nurse

Key Responsibilities:

• Assess and monitor physical, emotional, and spiritual needs of patients receiving palliative care.
  

• Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
  

• Administer medications and treatments for symptom control and pain relief.
  

• Educate patients and families on disease progression, care options, and end-of-life planning.
  

• Provide psychosocial support and counseling as needed.
  

• Coordinate care with physicians, social workers, chaplains, and other healthcare professionals.
  

• Advocate for the patient's wishes and ensure dignity and comfort are maintained.
  

• Maintain accurate and timely documentation of care provided.
  

• Participate in family meetings, care planning conferences, and case reviews.
  

• Support bereavement care for families after patient death, when applicable.
