Screening & Functional Assessment (SFA)
Conduct baseline functional assessments for identified CwSN.
Ensure proper documentation and referral planning.
Monitor the quality of assessments across designated blocks.
Therapeutic Interventions
Develop rehabilitation plans in coordination with PT/OT/ST/Psychology professionals.
Provide direct therapy sessions and home -based guidance.
Train parents and caregivers for continuity of therapy at home.
Develop modules, flyers, and training materials for stakeholders.
Medical Support
Organize medical camps in coordination with block -level teams.
Coordinate with specialists (Orthopedic, Pediatric, Neurology, Ophthalmology).
Ensure follow -up and implementation of medical recommendations.
Aids & Appliances (AAT Camps)
Coordinate the measurement, procurement, and distribution of assistive devices.
Facilitate linkages with DDRC, ALIMCO, and the Education Department.
Ensure follow -up on the effective usage of aids and appliances with children and parents.
Monitoring, Documentation & Team Collaboration
Maintain detailed records of assessments, interventions, and trainings.
Submit monthly, quarterly, and annual reports as per project requirements.
Document and share case studies, best practices, and key learnings.
Promote collaboration and coordination among team members for smooth program implementation.
Strong understanding of School Functional Assessment (SFA) tools and procedures.
Ability to identify, assess, and document functional limitations among CwSN (Children with Special Needs).
Knowledge of developmental milestones, disability classifications, and rehabilitation frameworks.
Proficiency in therapeutic planning and intervention for children with diverse disabilities.
Hands -on experience in physiotherapy, occupational therapy, speech therapy, or coordination with related professionals.
Ability to develop Individualized Rehabilitation Plans (IRPs) and therapy -based home programs.
Skills in training and capacity -building of parents/caregivers.
Strong interpersonal and communication skills to interact with children, families, schools, and multidisciplinary teams.
Coordination with medical professionals, specialists, and government departments (DDRC, ALIMCO, Education Dept.).
Ability to facilitate meetings, workshops, and review sessions effectively.
Competency in record maintenance, data entry, and report writing (monthly/quarterly/annual).
Knowledge of MS Office tools (Word, Excel, PowerPoint) for documentation and reporting.
Skill in preparing case studies, best practices, and knowledge -sharing materials.
Ability to plan, organize, and monitor program activities (medical camps, AAT camps, trainings).
Skills in quality assurance of assessments and interventions.
Experience in field coordination and community -based rehabilitation programs.
Capability to design training modules, IEC materials, and awareness flyers.
Conducting orientation and sensitization sessions for teachers, parents, and community stakeholders.
Ability to analyze functional assessment data and derive actionable insights.
Innovative in developing solutions for accessibility, therapy adherence, and inclusion.
