Job Description - Social Worker

Conduct psychosocial assessments of beneficiaries and their families.

Develop and implement individualized care and support plans.

Provide counseling, guidance, and emotional support to beneficiaries and caregivers.

Coordinate with multidisciplinary teams to ensure holistic care.

Conduct home visits and community outreach programs when required.

Assist beneficiaries in accessing government schemes, financial assistance, and community resources.

Maintain accurate case records, reports, and documentation.

Organize awareness programs, support groups, and community engagement activities.

Advocate for the rights and well-being of beneficiaries.

Participate in team meetings, training programs, and case discussions.

Ensure compliance with organizational policies, ethical standards, and confidentiality.
