Overview

Our client in the Connaught Region provides a comprehensive range of mental health services and community health services to the populations of Connaught, mental health services and some community services to a nearby community hospital, and a number of specialised services (e.g. Forensic, Eating Disorders) to adjacent counties and beyond.  The trust also provides CAMHS and eating disorder services to Galway, Mayo, Roscommon, Sligo & Donegal

The Trust’s clinical services are organised on the basis of care group clinical directorates:  Children and Young people, Adults, and Older Adults. All clinical services are developed in line with national policy guidance.  The service models adopted by the Trust support a strong focus on flexible community provision by comprehensive multi-disciplinary services backed up by effective day services and inpatient facilities.  In common with many other mental health trusts, the Trust’s use of inpatient facilities has been streamlined progressively in recent years allowing a significant reduction in bed numbers. 

Medical staff have contributed significantly to service developments, service redesign and efficiency programmes within the Trust, and to the related development of primary care mental health services.  An active programme of organisational and service development is well under way.  This supports coherent care pathways through primary care, secondary care and, when necessary, specialist clinical services and services provided by other agencies. Consultant psychiatrists are encouraged to contribute to this developmental work.

MENTAL HEALTH SERVICES IN CONNUAGHT

The total populations served by the Trust are approximately 527,185 in Connaught and 690,000 in Munster region. Both counties are regarded as prosperous with relatively low socio-economic deprivation and associated morbidity indices. However, this general picture disguises significant pockets of deprivation and associated psychiatric morbidity in the urban centres. These include ethnic minority communities and significant numbers of refugees in some settings.

The gender split for the county is similar to national and regional averages, with 50.9% females and 49.1% males. The age profile for the county is also similar to the national average at most ages. The largest difference is among those aged in their twenties, with lower proportions in these age groups compared to nationally. The under 19 population is 121,479, with 10 % living in poverty and about 400 looked after children at any one time.

Our client has been providing a comprehensive CAMHS service in Connaught Region for 7 years. During 2014 the service was offered for tender and the trust was successful in securing further seven year contract. The new service is based on the ‘Thrive Model’ and during October 2015 service transformation away from the traditional CAMHS tiered model to a service strongly influenced by children young people, their parents/carers and stakeholders began. The new model is based on ease of access, getting advice and consultation and support and, specialist clinical pathways.

Local HSE commissioned tier 4 inpatient facilities are within the trust and comprise The Adolescent Unit in our clients group. Forensic CAMHS is provided by a specialist regional tier 4 team, both supporting tier 3 and 4 CAMHS consultants, providing consultation and treatment to the YOT and other agencies.

Clinical Responsibilities

The post holder will join a multi-disciplinary team and take a leading role in providing psychiatric input to the assessment and treatment of children, adolescents and families referred there.  This will include face to face assessment and treatment as well as consultation to other team members. The post holder will liaise as necessary with hospital departments including A & E, Paediatrics, Community Paediatrics, Adult Mental Health and others.  The post holder will also liaise with other agencies dealing with children, such as education, social services and primary care. The liaison role will include strategic role in service development. Each of the psychiatrists will have a link with the localities to aid communication with GP’s in those areas

Cross cover will be expected between the consultants in this CAMHS team, and on occasion more widely within Connacht CAMHS, for CPD, study leave, annual leave, short sickness absence, etc.

The consultants within the team will determine, with other senior clinicians, Clinical Director and clinical team manager, how responsibilities will be divided between them.

Special interest sessions will be discussed within the aims of the Directorate and agreed with the Clinical Director and Service Director

Teaching Commitments

· regular commitments to ward based teaching of undergraduates.

· participation in the lecture programmes for 2 nd and 4 th year medical students.

· teaching of other groups as required including other specialty juniors and specialist nurses.

Job Plan A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Consultant Psychiatry- Child & Adolescent , on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

IMPORTANT GENERAL NOTE

It must be accepted that the resources available to the Trust are finite and that all changes in workload and developments requiring additional resources must have prior agreement through the Trust's Management arrangements.

On-call Responsibilities

The post holder is expected to join the CAMHS consultant on-call rota covering the Connaught Region. The frequency of on-call duties is 1:11. Category A 3% intensity. Most nights there is a Speciality Registrar or Registrar available, a senior mental health practitioner on each night takes the first calls.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Psychiatry.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
