Overview

A vacancy has been created for a consultant psychiatrist to provide clinical and managerial leadership as

part of the CRHT. This has partly arisen owing to restructuring of the hospital in the Leinster region, part of the Mental Health and Wellbeing Service.


CRHT – Crisis Resolution Home Treatment

The post will provide clinical leadership as one of the consultants in the CRHT. The CRHT team is now well established with several doctors and a 0.8 WTE consultant. There are several trainee doctors in the team (including higher and core trainees, GPTS, innovative GP trainees). The post could be considered for a job share.

The post holder will support the CRHT in multidisciplinary assessment, treatment and management of patients with mental disorders as envisaged in HSE Guidelines. CRHT covers the geographical area of only certain areas in Leinster. The service covers a borough population of approximately 208,000 and includes GPs working in 28 practices. The CRHT work in close partnership with the Dublin CRHT who offer support to patients living in the rest of Leinster. The post holder will liaise with the other relevant organizations. Professional accountability will be to the Medical Director and operational accountability to the Chief Executive Officer.

The workload in CRHT is approximately 40 patients on the case load at any one time, and the team comprises a well-supported MDT (details are given below of the MDT).

Candidates must hold either a CCT of the HSE’s specialist register with evidence of relevant training and experience. Full registration with the HSE and a driving licence are also essential. The successful candidate will be subject to appraisal and validation process according to HSE policies and procedures.

Candidates without IMC registration at the time of shortlisting will not be considered- as per hospital requirements.


Additional Information


You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.


Specific Duties

· Take on Consultant responsibility for patients in the crisis and who are under the care of the CRHT and provide consultant leadership to the multidisciplinary community team which includes:

0.8 WTE consultant, 1.4 WTE Associate specialist, 1 Higher Trainee and 1 core trainee. The post advertised will be an additional consultant post to the one that currently exists.

· Provide comprehensive assessment, diagnosis and management of issues relating to mental and physical health for adults with acute and severe mental illness in the community (over 18 years).

· Undertake all work in the context of the recovery approach which is emphasised across the hospital, placing the service user at the centre of planning and decision making.

· provide high quality, responsive, robust senior medical input into the mental health care of patients under CRHT

· supervise junior medical staff

· develop the service with a view to achieving accreditation at the earliest opportunity

· work closely with senior clinical and managerial staff to ensure bed occupancy in the area and remains at optimal levels

· provide reciprocal cover under Section 12 of the Mental Health Act for the leave, study leave and sickness of other senior medical staff in the directorate-it is currently 1 in 23 approximately and cover the catchment area.

· participate with other Consultants in providing section 12 cover for the region during the day-time

· encourage and lead on service based research and teaching

· participate in the framework of Clinical Governance as it develops in the Hospital Directorate and across the service

· participate in and contribute to service audit and evaluation

· liaise with General Practitioners concerning service users under the care of CRHT to improve overall patient experience and care

· attend and contribute to the local programme of audit, academic, management meetings

· participate in annual appraisal and job plan review with the Clinical Director or their nominee

· carry a mobile during work hours and be contactable by members of the team

· meet with the Clinical Director for annual job-planning

· engage with appraisal for revalidation, managed by the Associate Clinical Director for the service on a yearly basis

· recognise and contribute to safe practices in accordance with the Health and Safety at Work Act and with HSE policies

· The post-holder will also be involved in the regular consultant duty rota (approx 1:9).

· Work closely with colleagues from other teams including the psychiatric liaison service, community mental health teams, in patient services, substance misuse services, court liaison and diversion teams to ensure that CRHT provide a collaborative approach to the care of patients who are jointly under the care of these services. Develop appropriate referral pathways with these services to ensure collaborative working between different teams working ensuring good continuity of care for service users.

· The case load is 35-40 patients. There are on average 8 referrals per day, which can be assessed by members of the MDT. It is envisaged the referrals will reduce as the primary care services are enhanced.

Teaching Commitments

· regular commitments to ward based teaching of undergraduates.

· participation in the lecture programmes for 2 nd and 4 th & 5 th year medical students.

· teaching of other groups as required including other specialty juniors and specialist nurses.

Job Plan A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Psychiatry:  Crisis Resolution Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

A standard whole time job plan will contain 10 Programmed Activities each consisting of 4 hours. For

this post there will 7.5 PAs dedicated for direct clinical care and 2.5 Programmed Activities for

supporting professional activities. These Programmed Activities are subject to formal appraisal together

with the job plan. The job plan will be developed further in consultation with the Clinical Director. Job

planning will occur yearly but can be reviewed at any time at Consultants request

There is a weekly Trust wide teaching programme held on a Wednesday afternoon in a convenient

Location for all doctors. All doctors are encouraged to attend the teaching programme and a certificate of attendance is provided as evidence for CPD at the end of each term.

There will be opportunities to provide teaching for staff in other disciplines.

The post holder is expected to maintain good standing with the Royal College of Psychiatrists and maintain licence to practice from the HSE through successful revalidation arrangements. The post holder is encouraged to join one of the established PDP groups run in each locality.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Psychiatry:  Crisis Resolution

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

Service Provision and Cover during Absences

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.
