Cork Service


Fixed Term 6 Months – 35 hours per week


Salary €37,109.46 per annum




 


1. Role


 This role offers a fantastic opportunity to be a member of a multi-disciplined team using holistic approaches to support Depaul service users.  Working within a trauma informed principal of harm minimisation, where residents receive a high degree of care, support and dignity, Depaul has a commitment to service user participation and to empowering residents to manage their own lives. This role works across a 7-day week rota, and commences with increased annual leave days and other added benefits.


 


2. Key Areas of Responsibility



  • The core pillars of work for this service are:

  • Take a lead role in the Case Management of Service Users who present with complex needs.  

  • In partnership with service users, to assess their needs and develop workable support plans, with regular reviews on the progression of agreed goals.

  • To advocate effectively on behalf of service users and refer them to support agencies.

  • To work with partner stakeholders collaboratively, to meet needs of Service Users whose complex needs may fall across several multidisciplinary services or agencies.

  • To work with affiliated Depaul services to ensure optimum continuity of care.  

  • To report on work to team and partner agencies.

  • To ensure all resident files are kept up to date.

  • To work from a strong housing-led approach to support service-users in securing move-ons to the most suitable and sustainable long term accommodation.

  • Life Skills – Assessment and Practical Support - Building on existing life skills interventions to empower individuals in a more independent setting.

  • Relational Security Support – Managing risks relating to service

  • Supporting residents - To maintain positive Mental Health

  • Maintain of relationships with local community residents, organizations, businesses and relevant stakeholders. Support in planning, development and implementation of positive changes in line with expectancies.

  • Participate in existing local forum and share minutes and actions with local and senior management team and escalate issues as appropriate.

  • Ensure Depaul’s Good Neighbourhood Policy is being implemented within the service and support service users in adhering to policy expectancies

  • Interagency working with Partner agencies

  • Engagement with statutory agencies (Local Authorities/CWO etc.)

  • Managing service related risks

  • Ensuring excellent health and safety standards are maintained


The above list is not exhaustive; additional areas of responsibility may be added over time and flexibility to cover for other staff roles is required from time to time. 


 


3. Person Requirements



  • 2 years paid experience working in a social care field, desirably in a residential or homeless setting (essential)

  • At least one year of regular use of IT systems with a good proficiency in Excel & Word (essential) 

  • A qualification in a social care field, e.g. Diploma in Social Care, Diploma in Applied Social Studies, Diploma in Social Care Practice is desirable.

  • Good working knowledge of risk assessment processes and systems (i.e. S/U Risk assessment, Lone Working, Activity Risk Assessments) 

  • Process an understanding of harm reduction and low threshold working

  • Process an understanding of why people become homeless and the issues they present with, understanding what supports can be offered in homeless services to service users with complex support needs

  • Have experience working with needs assessment, keyworking and support planning for people with complex needs.

  • Have a knowledge of the statutory and voluntary sector resources available to homeless people

  • Understand the importance for Health and Safety in a residential setting

  • Be a good communicator and computer literate 

  • Knowledge and understanding of Depaul vision mission and values
