Job Description - Community Oncology Nurse

Oncology Community Outreach Nurse 

(Full time or part time roles available) - Community Oncology Outreach Services; South Dublin/ Leinster Region

Work as a Community Oncology Nurse for a Homecare company who are a leading provider of Homecare Nursing Services on a national scale. This is a community-based role covering Leinster.

In this position as a Community Oncology Nurse, you will work as part of a dedicated oncology nurse team. You will be responsible along with other team members for the delivery of Nurse Led oncology services. The post holder will deliver a range of specialised care and treatments to oncology patients in an outreach clinic setting, or within patients' homes. . This is an excellent opportunity to become a key team member in an innovative, and rapidly expanding service.

Responsibilities:

  • Under the leadership of the oncology clinical lead nurse, this role will involve supporting patients for phlebotomy services, nurse led toxicity assessments, IV anti-cancer therapies, oral chemotherapy assessments, chemotherapy pump disconnections, Central line care, IM/SC injections and more.
  • Accept responsibility for the patients referred to the outreach service and ensure the patient understands the required intervention.
  • Build relationships with all members of the referring centres multidisciplinary team through regular email, phone, and face to face communication.
  • Screening of new patient referrals and prescriptions to ensure all new patients meet criteria for community outreach services.
  • Work closely with TCP pharmacy and hospital pharmacy to ensure correct medication protocols are adhered to.
  • Work to the NCCP and TCP Homecare protocols and Work Instructions to ensure safe delivery of patient care and services.
  • Work to high standards of clinical practice to ensure that patients accessing outreach oncology services have continued access to specialist care, knowledge, and expertise.
  • Maintenance of accurate electronic patient records and the timely deliverance of reports back to referring oncology team

 

Key Competencies:

        Nursing & Midwife Board of Ireland Registered General Nurse (NMBI)

        Oncology Related post-graduation qualification essential

        Cannulation & Venepuncture experience essential.

        Two years oncology clinical experience (Day ward experience preferred)

        High degree of self-motivation.

        Evidence of personal and professional development.

        Effective administrator, with a high level of attention to detail.
