Job Description - Palliative Care Liaison Nurse




RecruitNet International Ltd specialises in Healthcare recruitment for Domestic and Overseas candidates for Hospitals, Nursing Homes, Home care and Community care.







We are hiring Palliative Care Liaison Nurse into a Hospital in Dublin, Ireland.







Responsibilities



​

  • Provide specialist knowledge, clinical expertise, and high\-quality care to patients receiving palliative medicine services across inpatient and outpatient settings.

  • Assess, plan, implement, and evaluate holistic care addressing patients’ physical, psychological, social, and spiritual needs, ensuring ongoing review and adjustment of care plans.

  • Conduct daily clinical rounds for palliative medicine patients and participate in specialist palliative care clinics.

  • Act as a patient advocate, participating in multidisciplinary team meetings, family meetings, and discharge planning for patients with complex needs.

  • Liaise closely with primary care teams, consultants, and the wider interdisciplinary team to coordinate referrals and ensure seamless continuity of care.

  • Promote collaborative, multidisciplinary working across hospital and community services, supporting patients and their families throughout the care journey.

  • Contribute to the development and implementation of care pathways, policies, protocols, and best practice guidelines within the acute palliative care setting.

  • Maintain accurate, timely, and legally compliant documentation, ensuring relevant statistics and caseload data are collated and reported appropriately.

  • Participate in quality assurance, audit, research, and service evaluation initiatives, ensuring practice remains evidence\-based and aligned with national palliative care guidelines.

  • Support education and professional development by providing patient and family education, participating in staff training programmes, and engaging in continuous professional development.










  • Requirements

  • Registered with An Bord Altranais (NMBI) with no restrictions on practice.

  • Hold a recognised post\-registration qualification relevant to the specialist area.

  • Minimum of 5 years post\-registration nursing experience.

  • At least 2 years’ experience in the specialty area of Oncology and/or Palliative Medicine.

  • Demonstrated interest in acute palliative medicine and service development.

  • Clinically competent in the assessment and care of oncology patients
