Penerangan Pekerjaan - Healthcare Coordinator

The Health Coordinator is responsible for coordination of navigation activities based on a given assignment of service types, and works hand-in-hand with peer case coordinators and staff to provide quality customer care. The Health Coordinator facilitates communication between interdisciplinary members and expedites customer services. The Health Coordinator is dedicated to providing support to customers and managing customer cases, but is not responsible for any type of clinical care.

Roles and Responsibilities

  • Screens incoming admissions of customers for eligibility for support services and facilitate services from community providers as needed.
  • Facilitates the care of customers in their homes under the direction of the physician by conducting intake, needs assessment, planning, and reassessment of service needs.
  • Acts as a liaison between the attending physician, vendors and the client to ensure non-fragmented, seamless, and holistic care through day-to-day support.
  • Acts as an advocate for clients to create a trusting environment for learning, adapting, and healthy change.
  • Coordinates necessary arrangements and follow up on services for customers in a timely manner.
  • Responds to customer questions, issues and/or concerns and relay information to the concerned parties.
  • Securely and accurately documents all customer information and completes reporting requirements through summary reports for physicians.
  • Ensures that work assignment and information gathered from day-to-day work is not shared with anyone and maintains HIPAA standards of privacy and confidentiality.
  • Attends in-service trainings and meetings at the request of or with the approval of immediate supervisor.

General Responsibilities:

  • Collaborative practice with strategic partners to coordinate all aspects of case management.
  • Properly conducting case management including, but not limited to concise and timely documentation of all case-related information into the Case Management System, following-up on cases, reading case notes, recording case notes, reporting to the next shift (“rounds”).
  • Creating a “team-player” environment by assisting co-workers with cases.
  • Researching information helpful both to clients and operations staff.
  • Participating in seminars or other continuing education courses.
