Clinical Nurse Advisor - Growth-Minded Organization

1. Aids hospital staff and physicians in the care of patients with stoma, draining wounds, fistulae, incontinence, or potential or actual tissue trauma.
2. Provide home visits or hospital visits to newly discharged patients. 
3. Follow up with patients via phone calls and provide necessary support on their needs. 
4. Provides support and education to patients and care-giver, including pre and post – operative teaching and ostomy appliance trouble-shooting/ maintenance. Completes documentation of all education provided. 
5. Develop and implement plan of care for patient. 
6. Evaluates implemented interventions and modifies care according to evaluation. Documents interventions and modifications to plan of care. 
7. Performs and documents physical, psychosocial, and emotional assessments for all assigned patients. Completes documentation of interventions. Confers with all members of health care team, sharing information and observations of patient and/or patient's family.
8. Assist sales team in conduct product training to HCPs
9. Prepare and deliver clinical cases presentations to internal and external stakeholders
10. Ensure timely and accurate submission of nursing reports.

11. Adheres to proper hospital and/or company policies, procedures, clinical          pathways and accepted standards of Wound, Ostomy, and Continence              Nursing Board
