Ensure clinical appropriateness of prescriptions and are accurately dispensed to the correct patient every time:

• Check all medications supplied are according to appropriate prescription with accurate labels and correct physical medications.
• Counsel patient on proper medication administration & technique (where applicable) and advise on appropriate storage condition.
• Provide alternatives or suggestions on medication treatment where appropriate.
• Able to lead shifts and/or teams where required, ensuring compliance to policies and procedures.
• Provide high quality medicine information.

Ensure efficiency in provision of ward supply:

• Supply of Dangerous Drugs and Psychotropic Substances to internal departments.

Ensure medications are stored and dispensed appropriately to maintain the efficacy and quality
 

Ensure quality and sterility of medication:

• Adhere and prepare formulation in accordance to Good Manufacturing Practice.
• Check quality and/or sterility of final product manufactured is in accordance to prescription or formulation.

Ensure the provision of an efficient and safe clinical pharmacy service.

Job Requirement:

• Possess a Degree in Pharmacy from a recognized institution.
• Minimum 3-4 years of hospital practice and with clinical experience.
• Preferably to have Antimicrobial Stewardship Program (AMS) experience.
• Ability to demonstrate good clinical pharmacy knowledge skills, clinical reasoning and judgment when assessing patient's pharmaceutical needs.
