Specialist Nurse, Accident & Emergency

SUMMARY OF DUTIES

Responsible for rendering professional nursing care to patients within an assigned unit of the hospital. Such nursing care is an integral part of the individualized holistic patient care plan as directed by the Medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services.

MAIN DUTIES RESPONSIBILITIES

1.Understands and carries out the nursing care in line with Vision and Mission Statements, Goals and Objectives that suit the scope of the Nursing Services.

2.Works shift duties or as assigned by Unit Manager according to Unit’s Operational Hours.

3.Informs Unit Manager when change occurs in circumstances which affect standards of care.

4.Receives and handover report of patient, knows the case in detail:

•  Progress of the patient.
•  Ensures all doctors’ orders are carried out before shift change and on taking over case.
•  Any incident or problems encountered.

5.Visits every patient in the unit together with the team members on taking over of duties to:

•  Assess condition of patient, compare with report.
•  Re-plan nursing care according to assessment or evaluation
• Determines nursing priorities and plans patient care according to patient’s needs and staff availability

6.Provides individualized holistic care to patients using the Nursing Process approach with proper nursing documentation in patients files and that it is contemporaneous.

7.Reports all abnormal findings and progress of patients to the relevant doctors / consultants.

8.Conducts regular nursing rounds on all patients in the ward to ascertain for other additional needs; talk to patient/family members as an integral part of providing emotional support.

9.Follows doctors / consultants ward round and assists them in doing procedures when required.

10.Carries out orders of doctors / consultants pertaining to care of patients and delegates to other team members according to their assignments.

11.Liaises and discusses with the relevant doctors / consultants with regards to care of patients in the unit / ward.

12.Establishes and develops effective communication with all disciplines, patients and relatives, to achieve optimal patient’ satisfaction.

13.Attends and participates in unit meetings, dialogue sessions and hospital’s activities.

14.Participates in the health education of patients and their families in the ward.

15.Assists the Unit Manager in the administration of the unit / wards when assigned as shift nurse in charge.

16.Acts on behalf of the Unit Manager in setting standards and expectations in the quality of care.

17.Carries out duties and responsibilities of the Unit Manager in her absence as directed.

18.Ensure charges of equipment/consumable items into patient’s charges accordingly and checked for completeness.

19.Responsible for checking and documenting supply of sterile packs from CSSS and weekly checking for integrity of packaging.

20.Maintains and documents adequate supply of unit stock.

21.Carries out assignments scheduled by Unit Manager and provide feedback.

22.Contributes suggestions in periodic revision and upgrading of Nursing Policies and Procedures and relevant work instructions.

23.Checks DDA drugs with the off-going Nurse In-charge of the previous shift when in-charge of the present shift.

24.Participates in the care, custody and administration of medications in accordance with Hospital Policies and Procedures.

25.Checks daily and audits resuscitation trolley periodically as scheduled. 26.Ensure all staff complies with Prevention and Control of Infection including implementation of surveillance activity in the unit and evidence of involvement. 27.Maintain confidentiality of patient’s personal and medical history.

28.Operates / maintains specialized equipment assigned to the unit and provides assistance / guidance to new staff in their use.

29.Assists the Unit Manager in keeping accurate records of equipment, medical supplies and drugs in the unit / ward.

30.Checks that all equipment in the unit are functioning well and service according to PPM schedule.

31.Report to Bio Medical personnel if any faulty and takes necessary action accordingly.

32.Assists in maintenance of instruments and equipment by proper cleaning, handling, disinfect and utilizing as directed.

Role as Nurse  in Accident & Emergency

1.Attending ambulance cases of transfer in and transfer out of patient safely to/from other healthcare services when required.

2.Assist Unit Manager to overseeing the smooth running of A&E Services 3.Conduct initial assessment or triaging of patient before assigning patient to the appropriate zone.

• Knows the cases in detail
• Complete the triage form
• Inform Medical Officer on duty for any cases come for treatment at Accident & Emergency Services.

4.Conduct top to toe examination, identify abnormal finding, records in the triage form and inform to doctor regarding abnormality.

5.Determine priority and plan care according to patient records.

6.Identifying and manage life threatening condition together other team members and doctor.

7.Liaises on and discusses with relevant doctors/consultants with regards to care of patients in A&E Services.

8.Perform phlebotomy and set up intravenous infusion for adult and minor patient as necessary and as ordered by the medical officer/consultants.

9.Serve or administer medications safely and timely for A&E patients as prescribed by doctors/consultants.

10. Perform dressing or assist doctors/consultants in simple surgical procedure in A&E procedure room.

11.Receives and hand over report of every patients in A&E services.

• Any accident or problem encounter.
• Transfer and transport patient safely to other services or wards when required.

12.Maintain and ensure medical equipment is working and functioning correctly at all times.

• Assist with admission procedures and discharged of patient including making appointment with specialist clinic if necessary.
• Handle death cases and ensure safe transfer of the body to the mortuary.

13. Any other duties as ordered by Charge Nurse/Unit Manager:-

• This job description of medical assistance is subject to periodic review and maybe changed at any time in consultation with the employee.
• Performs any other job related duties not specified in this job description when assigned by immediate supervisor.

14.Performs extra duties when called upon especially in an emergency.

QUALIFICATIONS REQUIRED

• Diploma/Bachelor in Nursing
• Post Basic qualification in related field.

EXPERIENCE REQUIRED

• Registered with Nursing Board of Malaysia with a valid license to practice as a Registered Nurse.
• Able to write and communicate in Bahasa Malaysia and English Language
• Willingness to work on scheduled hours, shift, and weekends, public holidays, on-call and anytime when called during emergency.
