Job Description - Community Support Worker

K’aute Pasifika Trust works with local communities towards a shared vision of improved health and wellbeing. We’re a charitable trust based in Kirikiriroa/Hamilton providing health, education, employment and social services to Pasifika communities, and all other ethnicities who wish to access our services. We have a strong 25-year history of supporting people and their families to make meaningful changes that benefit their health and wellbeing, and helping create the kind of connected community we all want to be part of.

A fantastic opportunity exists for a compassionate individual to join our K’aute Pasifika Aiga (family) as a Community Support Worker. In this fulltime role, you will play a pivotal part in supporting our General Health Team with our Pacific Home-Based Contract. The ideal candidate will have a great understanding of the needs of Pacific Island people/communities. This will include:

  • Providing support to the health team in enhancing the health and wellbeing of individuals and Pacific communities within the Waikato region.
  • Provide the health information, education and advice for clients and their families.
  • Engaging whanau within a supportive cultural context.
  • Using, comprehensive family centred planning tools and needs assessment as a means of enabling whanau and their members to learn about their condition, develop goals, plans and action that will allow them to live to the fullest and function well.

To be successful, you must have:

  • A community health certificate Level 4 or experience in similar role.
  • Professional training in health care, social work or similar field.
  • Good communications skills.
  • Good computer skills.
  • Preferably speak a Pacific Island language, but this is not essential.
  • Understands Pacific Island and Maori values and protocols ie. Te Tiriti O Waitangi.
  • A great understanding of the needs of Pacific Island people/communities.
  • Committed to professionalism.
  • Full NZ driver’s license is required.
