Job Description - Care Coordinator - Positive Work Culture

We are in search of an enthusiastic Care Coordinator to join our knowledgeable team at Hauraki Primary Health Organisation in Thames, Waikato.

Te Puna Hauora Matua o Hauraki/Hauraki PHO services work closely with our GP practice partner clinics, Kaupapa Māori providers, Te Whatu Ora, Te Aka Whai Ora and community services to improve health outcomes for whānau.  

This role is to ensure that the most vulnerable in our Hauraki community irrespective of PHO or enrolment status, receive the care and support they need to keep them well. You will need great working relationships with General practice teams, NGOs, Iwi, Pharmacy, Thames hospital and allied health teams but most importantly you need to have demonstrated experience working with our most vulnerable who have the highest risk of poor health outcomes. You will be advocating for your clients to ensure all services are delivering wrap round care, in a timely manner avoiding readmission.

Skills and experience needed for the role:

• Registered health professional who has proven ability to work with most vulnerable whānau in our community. 
• Ability to bring many strands of health care delivery into one cohesive plan for whānau making what can be very complex simplified and functional. 
• Communication skills and relationship building are pivotal to the success of this role. 
• Ability to think outside the box and be agile in your response to whānau need. 
• Ability to see and maximise the strengths in whānau, organisations and communities.
• Excellent communication skills both oral and written is essential. 
• New Zealand APC registration and a driver’s license. 
• Medtech and Indici experience is desirable and/or work experience in Te Whatu Ora.
