About the role 



Hours of Work: 24-28 Hours per week, 11.30am-8.00pm, between Tuesday - Friday 



As our Community Health Nurse, you will provide compassionate, person-centred healthcare support to people experiencing homelessness, hardship, addiction, mental health challenges, trauma, chronic illness or other barriers to care. You will work alongside individuals and whānau to assess health needs, support early intervention, improve access to services and advocate for equitable health outcomes.



What you’ll do



  • Complete physical, mental health, addiction and social health assessments.

  • Record and monitor key health indicators including blood pressure, heart rate, oxygen saturation, blood glucose, BMI and alcohol withdrawal scales.

  • Identify health risks and chronic conditions, and support early intervention, referrals and follow-up care.

  • Help people access primary health screening and services, including smoking cessation, bowel screening, mammograms, cervical screening, sexual health, family planning and dental services.

  • Provide health education (individual or groups) on disease prevention, lifestyle changes, nutrition, exercise, stress management and mental wellbeing.

  • Support people in crisis, including those experiencing family violence, substance misuse, homelessness or mental health distress.

  • Maintain medical supplies, support health and safety audits, and monitor infection control practices.



About you



  • You are a Registered Nurse or Enrolled Nurse with a current practising certificate.

  • You have community health experience or relevant qualifications, ideally with experience supporting people with complex needs.

  • You understand, or are willing to deepen your understanding of, homelessness, addiction, mental health, trauma and marginalisation.

  • You build trust through warmth, empathy, strong listening skills and clear communication.

  • You bring sound judgement, initiative and the confidence to respond calmly in conflict or crisis situations.

  • You are culturally aware, respectful of diversity and committed to ongoing learning about Te Reo Māori and Tikanga Māori.

  • You hold a current full New Zealand driver licence.

  • You align with Christchurch City Mission values and Te Tiriti o Waitangi principles.
