Community Health Worker (Chronic Care Disease Management Programme)

About the role:

We have an opportunity for a Community Health Worker to join our Chronic Care Disease Management Programme team within the Waikato region. This programme is designed to support a Whanau Ora Integrated approach to the management of chronic disease.

The Community Health Worker will work under the direction of the Chronic Care Nurses and will be assisting with key services like:

  • Whanau Ora social assessment
  • Health education and promotion, including delivery of healthy lifestyle package.
  • Health screening and monitoring
  • Health service referrals
  • Social and health care support and follow up
  • Social and health care integration and coordination
     

To be considered for this role ideally you will need to have:

  • The ability to work with a defined caseload of individuals to enhance their community living skills and develop self-management of their health.
  • Active participation in communicating and liaising shared planning with the client, their whaanau, other clinical and non-clinical providers.
  • Experience in chronic care disease management or related fields is desired.
  • Well-developed communication skills and the ability to build rapport and positive relationships with a diverse range of people and cultures in the community.
  • Sound Knowledge of Te Tiriti o Waitangi, Te Reo Maaori and Tikanga Maaori whilst working in a range of Maaori settings is an advantage.
  • The flexibility to travel within the role.
  • Experience with Medtech Evolution preferred.
