Diabetes Community Coordinator – Māori and Pacific

About the Organisation

Diabetes New Zealand is a national Charitable Trust with 14 Branches throughout New Zealand.  Our purpose is to support and assist people affected by diabetes to better manage their diabetes; raise the awareness of diabetes and its complications; and to promote the early diagnosis of all types of diabetes.

The people in these roles will be working in partnership with The Fono. The Fono is a primary care practice which provides quality primary health care services to an enrolled population of 16,000 people across the Metro Auckland region. This role will be located in Henderson with a focus on Māori and Pacific populations.

About the role

We are seeking enthusiastic applicants wanting to offer support to people with pre-diabetes and diabetes, and their whānau.  

They will work with the wider team of Health professionals including GPs, Practice Nurses, a Psychiatric Registrar, a Clinical Psychologist, Mind Health Practitioner, Health Coaches and Whānau Ora Navigators.

A personal experience with diabetes, either lived or whānau member would be an advantage.  They will provide lifestyle education to patients and their whānau. Health professional colleagues and Diabetes New Zealand will provide training.

The Diabetes Community Coordinators will administrate and co-facilitate diabetes self-management education programmes with health professionals. Programme specific training will be given for this. Completion of training in adult education would be an advantage. 

This person will ideally have links to community groups and providers or be able to develop them.

The Diabetes Community Coordinator will be located in The Fono in Henderson. This role will usually be Monday to Friday but may be required to do some work outside of usual business hours.  

The Diabetes Community Coordinator role is that of a Health Coach. The successful candidate will receive formal training for this role.

The successful applicant will have 

  • a full clean New Zealand driver's licence
  • comfortable with the use of Te Reo Māori or a Pacific language
  • strong computer skills using Microsoft office and other programmes, a ‘can-do’ attitude and be able to demonstrate strong personal and professional boundaries
  • demonstrated commitment to the Treaty of Waitangi and culturally safe practice
  • a clean criminal record – Police check will be sought when employed
