Job Description - Kaiawhina (Pou Haapai) - Opportunity to Make a Difference

We are hiring an expert Kaiawhina (Pou Haapai) to join our multidisciplinary team at Raukura Hauora O Tainui in Huntly, Waikato.

About us:

Raukura Hauora O Tainui provide a large range of primary and community health care services within our communities. We offer services for all ages and backgrounds, delivered by qualified and experienced staff. As a Maaori health organisation, we have a strong Tikanga Maaori based approach to providing better health solutions for all. 

Our new Taurima te Paa Harakeke service (the Paa service) is an integrated, community-based, Maaori-led programme of support based around a traditional Maaori Paa. The Paa concept is a village of services and skilled practitioners who can support whaanau along the first 2000 days of their kahu taurima (maternity and early years) journey. 
 

About the role:

Provide whaanau with guidance, assistance, and support to navigate the health and social support system during the first 2000 days of their kahu taurima (maternity and early years) journey.

As a Kaiawhina you are the first point of contact for whaanau entering and engaging our service. Your role is to undertake in-depth assessments of whaanau needs and wellbeing aspirations, and then collaboratively develop and work through a Whaanau Care Plan. The Care Plan provides whaanau and the care team with a pathway to wellbeing. You will also be supporting the wider team with referrals, taking immediate action, and addressing issues of highest priority to whaanau and their wellbeing. 

In many instances you will work autonomously, booking and scheduling appointments with them either in our clinic or a community setting (including their home) and working on your own caseload of whaanau with active Care Plans. Depending on need, you will have anywhere between 30 – 60 active Care Plans with whaanau.

Our service is based in Huntly and is covering the localities of Thames-Coromandel, Hauraki, Northern Waikato, and Hamilton/Kirikiriroa. 

This is a fixed term role available from June 2024 - December 2024.

Essentials required for the role: 

  • Familiarity with the whaanau in your locality and ability to build trusting relationships.
  • In-depth knowledge of the care system available in your locality, especially services available to whaanau during their first 2000 days and the ability to engage at all levels of the system. 
  • Excellent interpersonal skills and ability to build relationships with clinicians and other members of the multi-disciplinary care team.
  • Sound understanding of tikanga Maaori. 

 

It would be advantageous if you also bring the following: 

  • Relevant Tertiary Qualification or Whānau Ora Certificate. 
  • Well-developed approach to problem solving. 
  • Be an Approachable team player. 
  • Excellent computer literacy and familiarity with Whanau Tahi platform. 
  • Hardworking with a strong work ethic. 
  • Ability to cope with tight timelines and work effectively under pressure.
  • Intermediate level of te reo Māori.
