Job Description - Kaiwhakahaere Manaaki | Care Manager

Who we are looking for:



We are seeking a Kaiwhakahaere Manaaki (Care Manager) for Hamilton, who wants to support the independence of our tangata (people we support) so they can have great lives, make choices about how they live and what they do. Reporting to the Area Manager, and being based at our National Office in Frankton, Hamilton, this role is pivotal for the continued growth of our service throughout New Zealand. This is a very busy role and requires the successful candidate to hit the ground running. This role is responsible for coordinating the assessment, implementation, monitoring and evaluation of individual care and rehabilitation plans. You will ensure that care and rehabilitation plan reviewed as required and will Ensure all the authorities and strategies for care recipients, are in accordance with the ID (CC&R) Act 2003, and/or Criminal Procedures (Mentally Impaired Persons) Act 2003 or as directed by the Director-General of Health. You will also need to fulfill functions and duties as set out in the ID (CC&R) Act so that the tangata thrive in a safe and happy environment.



What you will need to be successful:



  • A commitment to the delivery of the highest standard of responsive, tangata centered care and support

  • Experience in working with people with high and complex rehabilitation and support needs

  • Degree level qualification in Nursing and/or Social Work

  • 4 years' relevant managerial experience in disability/health or correction services

  • Knowledge and/or experience with ID (CC&R) Act and other relevant legislative requirements

  • Demonstrated knowledge of the legal concepts central to secure services for people with an Intellectual Disability

  • Excellent communication and relationship skills

  • A team player with strong organisation and time management skills

  • A down-to-earth disposition, work well in a team, a professional attitude and be able to relate and communicate with people across all levels of the organisation

  • A commitment to a tikanga Māori approach to their work

  • Knowledge of Te Reo Māori would also be an advantage



Qualifications and courses:


• Tertiary qualification is preferred and /or significant equivalent experience from within a relevant sector
• Full NZ driver licence
• Travel component (hui/inter-regional travel/training etc.)
• Ability to work remotely when required
• Clear Ministry of Justice Check
• Suitable medical assessment results which includes drug and alcohol screening
