He Kōrero mō te tūranga - About the Role
Are you an inspirational, passionate, and big thinking Nurse Practitioner?
We have a brand-new Nurse Practitioner role commencing to join our Community Geriatric Service on a permanent full-time basis. The service provides specialist geriatric intervention for older adults with increased frailty living in the Bay of Plenty.
Our approach is to be flexible, holistic, and responsive, working alongside other health care providers to support the delivery of responsive care to vulnerable older adults.
The team delivers a community-based service to high-risk older adults requiring specialist geriatric assessment in their own home (or in ARC) that have increasing frailty, Cognitive impairment impacting on health outcomes, falls, poly pharmacy and other Geriatric risk factors.
As we move towards the Health New Zealand system redesign, we are aware of the opportunities this provides to adapt and deliver services that are accessible and responsive to people with both acute and long- term health challenges.
