Practice Pharmacist

Full or Part Time – Fixed Term

Riccarton Clinic is an accredited Urgent Care Clinic and General Practice that provides a wide range of general practice services from Monday to Friday to assist our enrolled patients in managing their health needs.

 

We are seeking to employ a Practice Pharmacist in a fixed term position until 30th June 2025 as part of the Comprehensive Primary Care Teams initiative.

 

The primary purpose of this role is to provide care across the comprehensive primary and community team for people and whānau at greatest risk of poor health outcomes and to actively support hospital avoidance and frequent attendance at Emergency Department/Urgent Care facilities. 

 

The Practice Pharmacist will work closely with primary and community health services provider network including Hauora Māori and Pacific providers, Kaiāwhina, general practice team and other health and social service providers in the delivery of the functions below, to achieve improvements in access and health and wellbeing outcomes for Māori and Pacific people / whānau.

 

Core activities include: 

  • Medicines management and optimisation 
  • Clinical evaluation and management of medication 
  • Medication reconciliation 
  • Advise and support whānau understanding and experience of medication treatment, how it can affect their health and wellbeing 
  • Improving management of high-risk medicines 
  • Provision of medicines information and advice 
  • Medication utilisation evaluation, audit, and quality improvement to improve the delivery of best practice and care.

You will have

Essential:  

  1. Qualified pharmacist with current APC without restrictions 
  2. Demonstrate an understanding of the significance of and obligations under Te Tiriti o Waitangi, including how to apply 
  3. Establish and maintain positive working relationships with people across the practice team, Kaiāwhina, Māori and Pacific CPCT partners, wider community and secondary care providers, external agencies, and NGOs. 

•     Comprehensive understanding of the inequities in access and use of medicines in Aotearoa New Zealand and its causes 

•     Advanced written and verbal communication skills 

•     Excellent time management and organisational skills 

•     Excellent critical appraisal skills, being able to identify the best evidence-informed solutions to clinical and practice questions and issues. 

•     Current driver’s license
