Provider Support – Strengthen Primary Care Across Waikato

Te Puna Hauora Matua o Hauraki | Hauraki PHO is seeking a motivated and relationship-focused individual to join our Provider Support team. This role plays a key part in supporting general practices to deliver high-quality, equitable care to communities across the region.

About the Role

You’ll work directly with general practices to strengthen systems, improve service delivery, and support continuous quality improvement.

Key Responsibilities

• Build strong, collaborative relationships with general practice teams


• Support practices to use data effectively for planning and improvement


• Assist with audits, accreditation, and programme implementation


• Promote culturally safe and pro-equity service delivery


• Provide training and capability-building across practice teams


• Help practices make the most of digital health tools and systems

What We’re Looking For

Essential:

• Experience in primary care or health service delivery


• Strong communication and relationship management skills


• Confidence working with data and digital platforms (e.g. Medtech, Indici)


• A genuine commitment to equity and improving outcomes for whānau

Desirable:

• Understanding of Te Tiriti o Waitangi in a health context


• Experience with quality improvement programmes
