About the role | He kōrero whakamārama mō tēnei tūranga
You will be working as part of our CREST Nursing service as a Case Manager to provide personalised community-based assessments to newly discharged clients with a restorative approach. This involves identifying and resolving issues as and when they arise to maximise rehabilitation potential. This short term service aims to return clients back to their baseline level of function and if that is not possible, to plan for long term support.
This role is about working with our already established team of Nurses, Coordinators, Support Workers and other Allied Health within the community setting. As a Registered Nurse you will provide a broad range of care to clients in their own home such as:
This is a full-time role, which will mainly be Monday to Friday. Part of the role is providing a 6 day per week service to clients which includes nurses working some weekends on a rostered basis, and on call on some evenings.
