We are looking for a social worker to join a transdisciplinary clinical team in an innovative and forward-thinking community-based practice.  

• The ideal candidate for this role must have full SWRB registration and current annual practising certificate. 
• The ideal candidate must have a full NZ driver licence.
• The ideal candidate should be able to undertake brief solution-focused therapy, or Health Improvement Practitioner (HIP trained).
• The ideal candidate should have local community knowledge of available services.
• The ideal candidate is a member of ANZSW.
• You will be working as a salaried social worker in a busy community care medical centre with an enrolled population of 12000.
• You will work closely with nurses, nurse practitioners, nurse prescribers, general practitioners, physiotherapist, and clinical psychologists, all playing a part in delivering the best possible healthcare. We embrace this diversity so that we can share our complex cases and not isolate ourselves.

We are a grass roots, community supported practice, partly owned by a Charitable Trust.

We have a research focus, around the drivers of poor health and interventions that will improve health. 

• We have a project with Otago University and the Health Research Council (HRC) looking at transdisciplinary care in community practice.  
• We also have a leading-edge programme underway to develop new technologies to help us with patient diagnostics and data use integrated into patient management systems.  This programme will also look at the convergence of many disciplines and how they can work together to create better health for our communities.

Palmerston North has affordable homes, great schools and a University, walks to die for, and the most cafes per capita in the country.  It is close to ski fields, beaches, and wineries of the Wairarapa and Hawkes Bay.
