We are seeking a highly qualified nurse with both hospital and community experience and an interest in prosthetics, orthotics and rehabilitation.

We are seeking an experienced nurse with an excellent understand of local tertiary and primary services that are available, and the ability to create links with our other Te Whatu Ora district services.
 


As a Nurse with Peke Waihanga you will join a highly skilled, motivated and dedicated national rehab team. Your role will involve enhancing the potential function, independence and quality of life of patients through assessment, health promotion, coordination of care and participation in the inter-disciplinary team.

This is a permanent, part time role. The days and hours of work are flexible though we anticipate approximately 20 hours per week.

The key responsibilities of your role will be:

  • Provide holistic nursing assessment, treatment and facilitation of ongoing care
  • Provide proactive, timely and appropriate patient care
  • Review referrals, liaise with patients, families and health professionals as required to ensure appropriate, patient centred scheduling.
  • Have oversite of patients’ management of diabetes, dialysis, pain, cardiovascular diagnoses
  • Support patients to identify realistic individual goals taking into consideration their health and lifestyle needs.
  • Actively participate in inter-disciplinary team meetings and patient clinics to support prosthetic/orthotic device prescriptions, contribute to the relevant paperwork (including high technology applications) and ensure timely and accurate documentation
  • Deliver health education and health promotion focused on secondary prevention
  • Provide wound care as required during prosthetic/orthotic/physiotherapy appointments
  • Make timely and appropriate referrals to provide a ‘wraparound’ service to patients
  • Triage incoming calls from patients with problems ensuring the most appropriate and timely intervention.
  • Support patients’ need for services closer to home by attending and promoting regional clinics

This position may also involve some coordination of new patients coming into the centre, supporting the team to ensure they are seen at the right time and referrals to other services are made as appropriate. 

 


The ideal candidate will have an interest in prosthetics/orthotics and rehabilitation, a background in vascular, diabetes or orthopaedic surgery would be beneficial, along with wound care experience. You will need to have excellent communication skills the ability to build strong relationships with other health service providers.  The ability to maintain these networks will be key in the co-ordination of patient care.
