Job Description - Health Improvement Practitioners - Part-Time

Health Improvment Practitioners!  Could this be a brand new career move for you? Join the movement.  Be a part of this new and exciting wellbeing and population health-focussed programme. No lengthy assessments and complex therapy plans; no caseloads.  Provide whānau supoort via brief, targeted therapeutic interventions in and alongside of your GP clinic health team. 

  • Are you a registered health professional with experience in talking therapies? 
  • Do you want to make a difference to health and wellbeing aspirations for whānau in your community? 
  • Deliver wellbeing support where whānau are in general practice. 
  • Specialised training and support provided

  • Locations include GP clinics in South, East and Central Auckland and as far as Franklin district.

We are looking for people with a strong understanding of cultural safety who are motivated to support whānau with a range of needs. Some experience in talking therapies is preferred. 

This is an exciting role where you will be trained and supported to offer a diverse and impactful range of wellness support from the General Practice Clinic, or virtually as might be required. You will work alongside of whānau to help them build knowledge, skills and confidence to positively engage in their hauora journey. The role primarily utilises Focused Acceptance and Commitment Therapy (FACT) brief assessment and intervention.  You will be a part of a team where a range of health and social interventions are offered.

About you

  • A fully registered health professional under the Health Practitioner Competence Assurance (HPCA) Act 2003 with a scope of practice that is appropriate to working as a mental health and addiction clinician; e.g. a Registered Nurse, Social Worker, Occupational Therapist, Psychologist, or DAPAANZ practitioner. You may also be a Counsellor with a full NZAC membership who has NZAC accreditation to work within the field of mental health and addictions. 
  • Have experience delivering talking therapies such as ACT or CBT in a culturally safe manner 
  • Have experience in applying brief interventions in their practice.
  • Have a sound understanding of Māori and Pacific models of health and wellbeing
  • Have an understanding of supporting rangatahi (youth), parents and whānau. 
  • You are fully vaccinated against COVID in line with Primary care protocols in our PHO
