5 weeks annual leave after two years
Sick leave entitlement of 5 days will be available from commencement of employment and another 5 days at 6 months of employment. 10 days entitlement will be available at 12 months service thereafter.
About the role
WellSouth is looking to employ a Registered Nurse who is registered with an appropriate professional body to work alongside the general practice teaching, facilitating and promoting enhanced management of long-term conditions across the southern region.
The person in this position will support practices with implementation of WellSouth Long Term Conditions Programmes, and ongoing utilisation of the programmes including care planning and MDT meetings.
There is an ability to negotiate days/hours of work for the right candidate.
What we are looking for:
Have strong assessment skills and the ability to plan care based on assessments made.
Have 3-4 years' experience in a similar role and the ability to work with people from diverse cultural backgrounds
Be a Registered Nure, experienced in primary/community care and with a current practising certificate and a clean driver's licence
Have key skills and knowledge in long term conditions management with a focus on either Diabetes, Cardiovascular, Respiratory and Gout.
Have experience in preparing and delivering education to both patients and colleagues
Have proven strong relationships and interpersonal skills. The ability to quickly develop a rapport with patients and to genuinely care about their health. Have empathy and sensitivity with the ability to listen and quickly read and respond to the situation.
