Registered Nurse - Dynamic Team - Part-Time

About the role:
This is a case management/key working role, which includes, but is not limited to; assertive outreach, advocacy, service co-ordination and collaboration, risk assessment, and safety planning and working within harm reduction, strength-based and recovery models as well as utilising a motivational approach to support tangata whaiora to make significant changes in their lives.
You would be working in a well-supported team where you will develop unique expertise and skills to best support tangata whaiora experiencing the above issues. The role is Mon-Fri full time or part time permanent,

What you have to do:

• Provide screening, appropriate consultation and referral in relation to mental and physical health and wellbeing
• Identify needs, goals, risks, and early warning signs that will inform care plans and treatment interventions required
• within a harm reduction framework
• All practice interventions are managed in a safe and appropriate manner and according to best practice guidelines. e.g.
• administration of interventions, treatments, medications
• Health education is delivered in a way that meets the needs of the clients within a nursing framework
• AOD and other relevant education and information and advocacy is provided for family as
• appropriate

Skills and Experience:

• NZ Registered nurse with current APC
• Excellent engagement and communication skills
• Be able to work confidently autonomously and within a team
• Clinical experience in the mental health and/or addiction setting
• Full drivers license
