Join our clients team as an Enrollment Registered Nurse for the PACE Program in Los Angeles. This individual will help to bring the PACE (Program of AllInclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles. The Enrollment Registered Nurse is a key player of the Outreach Team as we find and assess prospective participants for our PACE program. This person acts as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process for the prospective participants.

Key Responsibilities

Collaborate with Outreach Specialist and IDT to ensure successful enrollment

Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care

Conduct home safety evaluations of the prospective participants home

Participate in Enrollment Eligibility meetings to review Level of Care recommendations

Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS

Maintain accurate documentation and record all evaluations in CRM software

Participate in all PACE Audits by DHCS and CMS

Support the Center Director to develop participant retention strategies

Maintain knowledge and expertise in eligibility enrollment and program specifications for the PACE Program

Support business development and census growth by participating and assisting with outreach programs as needed

Schedule and Shift Details

MF 8a5p. Mostly infield home appointments and occasional onsite work in Los Angeles clinic up to 5 days per month.

qualifications

• Associate degree or higher
• RN (3 years)
• 3 years of experience in ANY of the following:
• Geriatric Care
• Senior Care
• ALL of the following valid licenses/certifications:
• CA driver s license personal transportation good driving record and auto insurance
• American Heart Association Basic Life Support (AHA BLS)
• Registered Nurse (RN) in California (CA)
• Do you possess full proficiency in speaking reading and writing in Spanish (yes)
• Have you applied or have been previously employed at myPlace Health (no)
• Have you spoken to a Relode recruiter by phone about this position (yes)

• 1 years of experience in ANY of the following:
• In Home experience
• Enrollment
