Home Health Care Assistant

The Clinical Compliance Associate works directly with the Director of Compliance to ensure that clinical files remain compliant with the guidelines of all states operating under the agency’s license. This role is essential for maintaining accurate documentation and supporting compliance workflows.

Key Responsibilities:

• Verify completion of service tasks in the EMR and associated systems.

• Confirm accuracy and completeness of progress notes for skilled care cases.

• Ensure Plan of Care (POC) is completed within 24 hours of Start of Care (SOC).

• Ensure 485/Initial Assessment documentation is submitted within 72 hours of SOC.

• Follow up with Primary Discipline RNs to resolve documentation gaps.

• Collaborate with RN Care Managers to implement audit corrections.

• Monitor RNCM visit scheduling and productivity (minimum 3 visits/day).

• Track and report monthly visit counts and identify discrepancies.

• Monitor incident reports, fall follow-ups, and on-call logs.

• Confirm transfer forms and reassessments are completed on time.

• Ensure all state-specific documentation requirements are met.

•High school diploma or equivalent.

• Minimum of 2 years of professional experience in Community/Home Health, including home health regulatory compliance.

• Knowledge of HIPAA and regulatory standards.

• Experience with data collection and reporting
