Job Description - Health Practitioner

HIRING….
We are looking for experience for Health Practitioner/Health Coach/Community Health Coordinator to support our US team.

Location: Onsite/Residing Philippines
Employment Type: Full-time/Independent contractor
Duration: Long term

Responsibilities:

Act as a primary non-clinical point of contact for patients
Conduct wellness check-ins, follow-ups, and patient engagement activities
Provide basic health education, lifestyle guidance, and wellness coaching (non-diagnostic)
Perform community or field-based coordination, including local follow-ups and support as needed
Assist patients with scheduling, referrals, and navigating health or wellness services
Collect patient information and escalate clinical concerns to the Registered Nurse
Maintain accurate documentation and follow escalation and compliance protocols

Qualifications:

Background in health-related fields such as nutrition, allied health, public health, wellness, or community health
Strong communication and interpersonal skills
Willingness and ability to perform community or leg work when required
Ability to follow clinical direction and escalation guidelines
Comfortable using digital tools for documentation and communication

Preferred Qualifications

Certification or training in health coaching, nutrition, wellness, or community health
Experience working with barangay health programs, NGOs, or community outreach initiatives

Multilingual skills (Filipino, English, or regional dialects)
